Question: 39 woman 315lbs diagnosed with PCOS. I take lexapro for anxiety, omnapralzol (spell?) for acid reflux or something like that, medroxyprogesterone to bring on period every couple months. Previous fasting labs high liver enzymes, insulin high. What causes clear white milk like nipple discharge when I squeeze nipples? Both nipples do it sometimes

You need to get your prolactin hormone checked. Increased prolactin secretion( hyperprilactinemia) can cause milk secretion from the breast.
Usually women with PCOS have increased possibility of having decreased thyroid function ( hypothyroidism). This can also cause increased prolactin secretion resulting in milk secretion from the breast. So get your thyroid hormones tested.

The medications you are using for anxiety and acid reflux can also cause milk secretion from the breast. Discuss with your doctor and shift to safer alternatives.

my medications can cause that? Previous labs showed high liver enzymes (75), high insulin, normal thyroid and everything else. Can pcos cause? my age? I have 3 kids. Do I still have PCOS with normal vaginal ultrasound? Should I freak out?

Yes the medicines you are taking can cause milk secretion from the breast.
Which liver enzymes were high? and how high are they?
It is good to know that you have normal thyroid function. Check the prolactin hormone.
Yes one can have PCOD with normal ultrasound of the ovary.
There is no need to freak out. What is your BMI? First reduce your weight. Follow a healthy life style. Reducing the weight sets many things right for PCOD women. Do check your blood sugar by oral glucose tolerance test every year as women with PCOD are more prone to develop diabetes.

Your insulin level is high. It suggests that you are at risk of developing diabetes. Please get a oral glucose tolerance test done along with fasting lipid profile.

The liver enzymes are slightly high but not significant. This could be due to obesity too. Get started with your determination to reduce weight. Follow healthy lifestyle. Repeat the liver function tests after 3 months. Based on those results I can tell you whether you need further tests or follow up.
