What Causes Bloating, Difficulty In Swallowing And Breathing With Normal Endoscopy?

Question: Hi,

I had been having issues such as bloated stomach, feeling food in throat, difficulty swallowing and breathing difficulty in the last 3 months. I did a endoscopy 2 months back and doctor prescribed nexium 40 for 15 days. They said I had excess stomach acid and nothing else to worry. I was feeling better after that however i lost my apetite and not able to eat fully. I also started eating only roti, dosa and rice after that instance. Due to this I lost 6 kg weight in last 4 months.

Last week I had alchol, and since 3 days i am again having difficulty eating. Please note I do not have any other symptoms such as food in throat or difficulty breathing. I am unable to eat more than one dosa or one slice of bread. Even not more than a cup of juice. Feels like food may get struck or hard to digest.

I wanted to know If I can take Nexium 40 Again ? Can Nexium be used as and when needed basis? How long should i again take it?

I also read about esophageal stricture online and I feel I may have it as i have difficulty eating and swallowing. But I did a endoscopy 2 months back when i had worse symptoms and doctors didnt say anything about it. would the doctors have missed it?

If endoscopy is normal i guess there is nothing much to worry.
Worthwhile getting some basic tests like an Complete blood count, TSH, SGPT and creatinine done if not done earlier to rule out other causes of anorexia, weight loss.
Can restart neksium also can take an pro kinetic like domstal (available over the counter) 10 mg thrice a day for 2 weeks and see how you feel.
Some life style modification will surely help you.
Lifestyle modification includes
1) eating small frequent meals
2) avoiding sleeping 2 hours after meals
3) taking adequate water and fiber in diet
4) avoiding stress
5) taking adequate sleep
All this will help to control symptoms better.
IF you or not better worthwhile visiting your gastroenterologist again. Can reduce dose of neksium to 20 mg once better.
Unlikely the doctor would have missed any stricture, but visit a gastroenterologist if you do not feel better or otherwise after 15days.

Hi Doc,

Thanks for your prompt response.

1. Are the basic tests like an Complete blood count, TSH, SGPT and creatinine done via a blood test sample ? So a single blood test would be enough right?

2. Do you also suggest the test where the patient takes a liquid and then a xray to see the structure of esphogeous ?

3. So I should restart Nexium 40 for the next one week and then reduce it to nexium 20 right? In addition i should take domstal thrice a day right?

Right now I am only having swallowing difficulties and slight reigeration of food. I am finding it hard to eat more than one dosa / idly. Also need to chew for long before digest if not feel it may get stuck. Is this normal and will it get better over a period of time?

1) Yes they are all blood tests, single fasting blood sample should be good enough.

2) Well that's called a barium test - no need to do it right way can wait and see.

3) Yes can do that.

well bit unusual for a acidity symptoms bu can happen, can wait for some time and see how you feel, if not better can consider doing a barium swallow test.

You had asked me to take pro kinetic like domstal 10 mg for 2 weeks. Since this is a anti biotic, can I take it without any further tests?

I mean, what if I don't have the bacterical infection and can I still take this tablet along with neximum 40?

Domstal is not an antibiotic. You can take it.
Hence is has got noting to do with bacterial infection which you do not have.
