Posted on Wed, 8 Oct 2014
Question: My son has a tiny hole in his lower abdomen that was bleeding. He says it doesn't hurt or itch. It has a small bruise around it. He just noticed the blood on his underwear when going to the bathroom. What is it? What do I do? It's a little scary looking Thank you. M

Brief Answer:
The coagulation profile needs to be checked

Detailed Answer:
Hi,

Thanks for posting on healthcaremagic.

From the description it seems to be a bleeding from the umbilicus. Information on the amount of blood and a direct photograph would be helpful. Please revert back with the requested information and I would try to help further. I would probably request you for some blood tests like coagulation profile and platelet count at that point of time.

regards,

Hello, Thank you for your quick reply.

My son is a bit overweight and we have been searching online for answers.
I believe it may be a Pilonidial cyst from the many descriptions we've found.
Although generally found on the coccyx area, it seems it can also appear elsewhere in folds in the skin. Poor bathing hygene, overweightness, too much sitting etc. all seem to describe it. So who should we see for treatment? His pediatric M.D. or a dermatologist? I do not think the pedi will be able to treat it in the office. As it might need a local and some kind of abridement. They'll probably just refer us to a derm. The problem with derms is they usually make you wait 1-3 months for new patient appointments. What is your advice? The hole is not near the belly button. It's more in the groin area. Thank you. M

Brief Answer:
pilonidal cyst unlikely. photo to be helpful

Detailed Answer:
Thanks for writing back. It is unlikely to be a pilonidal cyst but it is hard to comment without examining. That is the reason I insisted for a photograph to be attached. A XXXXXXX image will also do for now. The exact location is also important which should be visible in the photo. If I get the details I hope that would be able to guide you on what to search for and which doctor to visit. As you should understand, I cannot do it blindly. Normally if debridement is required, it is fine by a surgeon but whether it is required is something I cannot comment without looking at it.
