Question: RVID cm.... 07-2.4 Syst Diast N LVID cm... 3.7-5.4 Syst 2.8 Diast 4.9 POST. WALL THICKNESS 0.8-1.1 Diast 1.0 SEPTAL WALL THICKNESS 0.7-1-2 Diast 0.8 LAID 1.9-3.8 Syst 2.8 AORTIC ROOT DIMENSION 2.0-3.7 Diast 2.7 MTRL. VLV. VEL. D.D.R. 80mm/sec-150mm/sec Diast N Comments: The patient underwent 2-D, M-Mode and doppler examination including s pulse-wave, continuous-wave and color-flow analysis; the study is technically adequate; the following is observed: AORTIC VALVE: normal; tricuspid in appearance with normal central closure. MITRAL VALVE: mitral valve prolapse. TRICUSPID VALVE: normal. PULMONIC VALVE: not well visualized. LEFT ATRIUM: normal; no intracavitary masses or clots seen. AORTIC ROOT: normal size with adequate motion. LEFT VENTRICLE: normal chanber size. Normal LV systolic function. Ejection fraction was 60%. RIGHT ATRIUM: normal. RIGHT VENTRICLE: normal size and function. PERICARDIUM: unremarkable; no evidence of effusion. DOPPLER: doppler examination of the aortic, mitral, tricuspid, and pulmonary valves and performed. This shows decreased compliance of the LV. Moderate mitral reguritation and mild tricuspid regurgitation. CONCLUSION: 1. Mitral valve prolapse. 2. Normal LV systolic function. 3. Decreased compliance of the LV. 4. Moderate mitral reguritation. 5. Mild tricuspid reguritation. I sent the results of my echo I had done two weeks ago. I typed it out because I don't have a copy machine. I am a 54 year old female. I am a non smoker and don't drink at all. I weigh 127 lbs and am 5 ft 6. I have a history of SVT, MVP, PAC's and PVC's. I have been on tenormin since age 22 for the SVT. It is 50 mg but I only take 1/4 at bedtime unless I have an episode of the SVT. I also take Celexa 10 mg at bedtime and Clonazepam 1 mg at bedtime in case I have an episode of the the SVT. I have swelling in the lower legs, in my calves and my feet, or ankles. Not the sides of them, more on top. I also have been short of breath and when I tell my heart dr he doesn't seem concerned. I see where I have moderate leakage and wonder if this could be causing my swelling and if it is serious. Also, I wonder if I have some sort of blockage in my arteries, or valves in my legs because they ache at times. To have a catherization, can it be done thru the arm instead of the groin area? Can a MRI angiogram or CT scan of the heart show blockage as well as a catherization?

I have a picture of my ankle to show you, but don't know how to send it. It says to upload it in image or pdf format in the right pane, but guess I don't know how to do that.

Ok, I think I figured it out and sent a pic of my right ankle tonight being it is swelled more than the left one. I haven't been up on it much at all today. It will be gone by morning, but will be back after I have been up for a bit. I would really like to know what causes it.

Ankle swelling may be due to many reasons like anemia, heart or renal problems, hypothyroidism etc.

In spite of moderate mitral regurgitation, your ejection fraction is satisfactory. Your ankle swelling may be due to heart problems, but it is not alarming.

You should take your drugs regularly. You should try to avoid standing for prolong period. Don’t hang your legs while sitting position; try to keep it in horizontal position while sitting.
Hypothyroidism should also be excluded.

CT Angiography can estimate block but conventional angiography is the best. Angiography can be done after proper assessment. Yes, catheterization it can be done through arm or groin.

When you say it could be caused by heart problems, what kind of tests would I need to find out if the swelling is from it? I have already had the echo. Couldn't the swelling be caused from going on in my legs, like vein pooling, or venous insufficiency? If it were from my heart, wouldn't both ankles be swollen, not just one? Also, what kind of heart problems would cause the swelling if it were from my heart? Why do you say it may be caused from heart problems, but isn't alarming? What kind do you mean that wouldn't be alarming? I also have a crease in my ear lope, and I have read many times this is an indicator for heart problems, or blockage. Is this true? One more question, what do you think would make me short of breath like I explained? Would it be the leakage?

Heart problem is indicated from many factors like ECHO report(tricuspid, mitral regurgitation etc)and present complain of breathlessness.

Heart problem generally causes bilateral swelling in lower limbs.I interpreted that you have bilateral swelling,more in right ankle.
It is good that in spite of findings in ECHO, contraction of your heart is alright.

In unilateral swelling,cellulitis and thyroid dysfunction should also be excluded.

Venous insufficiency causes dilation of superficial veins and this can be seen by naked eyes.Superficial veins becomes dilated and tortuous.So,it is unlikely cause in your case.

Pedal edema can be managed by posture management and drugs like diuretics.

Crease in ear lobe is not related to heart failure.

Radiologists are also doctors.Radiologists are specifically trained in imaging modalities like Ultrasound,CT scan ,MRI,ECHO,X-rays etc.

So are you saying you think the swelling and shortness of breath are caused from the tricuspid, mitral regurgitation that showed on the echo? When you say heart problems generally causes bilateral swelling in lower limbs and that you interpreted that I have bilateral swelling,more in right ankle,what exactly does this mean? Does it mean the leakage is causing the swelling and if so, why does it cause it and why only in one ankle or leg more than the other one? Also you say pedal edema can be managed by posture management and drugs, what would cause this pedal edema, the leakage? I can't sit all day long and prop my legs up, and if I am up for only an hour, the swelling begins. If I have blockage in my legs or it is from deep veins that can't be seen on top, can't the pain and swelling be caused from maybe circulation from that or the valves in my legs not being good, because they do ache?

You need proper clinical examination.Some times mild pedal edema can't be seen by naked eyes.It should be assessed by proper clinical examination.Ankle at the level of bony prominence(medial malleolus)should be pressed for 30 secs and then it is palpated for edema.
Breathlessness with pedal edema indicate more towards heart origin but other causes should also be excluded as discussed earlier.

Venous insufficiency can be evaluated by colour doppler of leg veins and clinical examination.

Do you think the breathlessness and edema would be caused from the leakage or what other heart problems would cause this? If not the leakage, what other heart test would you suggest I have? What other causes should be looked for? You need proper clinical examination. Your ECHO finding suggests incompetent valves in the heart.Incompetent valves is causing regurgitation(back flow of blood ).

First it should examined by a clinician that you have unilateral or bilateral edema.Clinical method is discussed earlier.Bony prominence in medial side of ankle is pressed for 30 seconds then depression is felt by a finger.

Causes are different for unilateral and bilateral pedal edema.
Unilateral edema is generally due to cellulitis,grave's disease,incompetent leg veins etc.
Where as bilateral pedal edema is commonly due to heart,renal problem etc.

On the bony part of my ankle, I can't see any swelling. You can tell by the picture I sent you that it is on top of my foot and not beside on the ankle. Also being I have swelling in my lower legs and they ache do you still think it is from my heart? And when you say the swelling can be caused from the heart, is it because of the leakage and is that why I am short winded? Should I be concerned with the swelling in my legs. It does bother me, because they look swollen at times at the lower part and they ache and throb?

Cardiac origin is one of the possibility.However,other causes should also be ruled out.
I can see little bit of redness over the swelling.Inflammation/cellulitis should also be ruled out.
Ache is due to fluid accumulation.
Since you have respiratory problem,you need proper respiratory examination including chest X-ray(PA view).
Colour doppler of lower limb can be done after evaluation.

I have had chest x-rays, what other test do I need for the shortness of breath? If I am short of breath from my leaking valves, don't I need to have surgery? And why at times am I not short of breath? Seems it comes and goes. The reason for the redness is because I had been sitting on my foot and it was a bit red.

It is important to know the findings of chest X-ray.If there are features of fluid accumulation then diuretics can be prescribed.Chest auscultation is also very informative.
You don't need operation at present.
Breathlessness due to fluid accumulation is more in certain postures like lying flat while it get relieve in propped up position.
