What Are The Side Effects Of Piopar MF Taken For Diabetes?

Question: iam aged 51 and diabetic since the last 10 years. my height is 164 cms and weight 56 kgs. there is a family history of diabetis. I have been prescribed Piopar MF 7.5 twice a day. what are the side effects and is it safe.

Thanks for your question.
Noted your concern.

You have maintained a healthy BMI (Body Mass Index) of 20.8

After going through your AGP (Ambulatory Glucose Profile) there is more problem with pre-meal blood sugar levels (especially fasting) than post-meal blood sugar levels.

Whatever changes in medications done in your current prescription are quite acceptable.

Tab Galvus contains Vildagliptin which help in reducing insulin requirement (decreases insulin resistance) also stimulates pancreas to secret insulin which is glucose dependent (ie. insulin secretion in response to elevated blood sugar level only).

Tab Piopar MF contain Pioglitazone and Metformin.
Both these drugs reduces insulin resistance with different mode of action.

All these three drugs rarely cause hypoglycemia when used alone or even with use in combination with each other.
Risk of hypoglycemia with previous prescription containing Tab Ziglim (contains Glimipiride) is definitely more as compared to current prescription.

In the presence of normal Kidney function denoted by normal serum creatinine and eGFR (Glomerular Filtration Rate) and normal heart function denoted by Ejection fraction on 2DEcho of heart all these drugs are relatively safe.

Use of Pioglitazone may be associated with weight gain, swelling over legs, some cases of osteoporotic fracture reported with its use (fracture risk is more in female as compared to male.)

So it is advisable to do regular weight monitoring, to see any leg swelling, check Vitamin D3 levles and correct it, if low.

In such a scenario as you have described, I may have suggested to do HbA1c and depending upon its value and considering normal heart and kidney function may have increased the dose of Metformin from 500 mg to 1500 mg per day in divided doses, may have stopped Tab Ziglim and may have used Vildaglitin 50 mg twice a day.
