Brief Answer:
All reactions to metformin and clomid

Detailed Answer:
Hello and welcome.

Thank you for writing to us.

I am guessing you have a history of irregular periods, and also are trying to lose weight (if you do not mind me asking), right?

Well, the medications that our previous doctor has prescribed are for the treatment of polycystic ovarian syndrome (PCOS). Metformin helps in regularizing the periods as well as promoting weight loss. Due to this medication, you might be experiencing a drop in appetite, which should help in your weight loss. This reduced appetite might have resulted in a reduced intake of food that has caused the gassy feeling in your stomach. If you were my patient I would add an antacid or histamine H2 receptor antagonist to your treatment regimen; but if you are trying to lose weight, then I would not discontinue the metformin.

The clomid (clomiphene) is used to increase fertility in women diagnosed to have PCOS, and are trying to conceive. Are you trying to conceive ma'am? This medication can cause mood changes, that has resulted in your drop in libido, if you are not trying to conceive then you may discontinue this medication.

I hope this information helps ma'am. Do write back to me for any further clarifications, I would be more than happy to help.

Take care.

Brief Answer:
First regularize periods, then try to conceive

Detailed Answer:
Hello once again XXXX.

Thank you for that information, it does help.

I would like for you to first understand that treatment is a step-by-step process, so we will have to tackle each obstacle one at a time.

For this, we will first have to regularize your periods and then move forward with help you to conceive. Metformin is a good medication that is usually prescribed for regularization of the menstrual cycle, as well as weight loss. But, till off late you have not yet regularized your menstrual cycle, this might take time.

If I were your treating doctor, I would shift over to a course of oral contraceptive medication for about 2-3 months. During this period, you will have to discontinue the clomid. After 2-3 months of oral contraceptives, which should help bring your menstrual cycle to a normal or close to normal state and increase the chances of conception; we can then initiate clomid along with hCG injections.

I would thus request you to discuss the same with your treating doctor, and go forward with this. The use of oral contraceptives for 2-3 months increases the effectivity of clomid and will help in rapid and quick conception.

I hope this information is clear and helps you ma'am. Do write back to me for any further help. I wish you the best and nothing less.

Take care.

Brief Answer:
Continue Metformin, start clomid after 2 cycles

Detailed Answer:
Hello XXXX.

As you have just started metformin, we will have to wait for a response. It is too early to say that metformin is not helping. Metformin is indeed a very good medication, and if it does help in regularizing your periods, then we might not even need to use oral contraceptives.

I would advise you to give metformin some more time, and initiate clomid only after 2 cycles (which means the cycle after the next).

Do keep in touch and keep me updated. I shall let you know when the best time to start clomid is, and also let you know when you should visit your doctor for other conception-assisting medication.

Wishing you a speedy normalization of your periods and a successful conception soon.

God bless.

Brief Answer:
My direct link and medication for gas

Detailed Answer:
Hello XXXX.

I can understand it is difficult to keep explaining history and so on to each new doctor, which is why from now onwards you can just click on the below link and directly ask me questions without having any trouble of explaining your history and so on. My link is: http://doctor XXXXXXX com/doctors/dr-shoaib-khan//0000
Just click on it and you can ask me a question directly.

For the gas you can initiate a histamine H2-receptor antagonist (eg. Ranitidine) to be taken 10 minutes before a meal anytime you have gas trouble.

Also eat small meals (5-6 small meals, instead of 2-3 large ones), but eat regularly, do not keep your stomach empty and drink plenty of water

I hope this helps. Travel safely and do keep in touch with me about details of your menstrual cycle, please be patient I am sure we will get through this.

Take care.

Brief Answer:
Normal on first or second day of period

Detailed Answer:
Hello XXXX.

Hope you have been well.

A dark brown/blackish vaginal discharge on the first or second day of a period or during the last 2 days of period are considered normal provided you do not change your pad more than 7-8 times a day; each time for 6-8 drops of blood.

As far as your presentation does, your menstrual cycle is indeed still irregular, but I would not really worry about the dark brown/blackish blood as this was seen on the first or second day of your period. I am sure you are fine.

Take care and keep in touch.

God bless.

Brief Answer:
Metformin 1000 mg dose is correct, no changes

Detailed Answer:
Hello XXXX.

A clinical response to metformin prescribed to help normalize the menstrual cycle in a woman diagnosed to have polycystic ovarian syndrome (PCOS) is seen at 1000 mg per day.

You are taking metformin 500 mg twice a day, which is 1000 mg per day. Please continue with this dosage, do not change it. The response might be slow, but should be seen within the next 2 months.

Wishing you the best in life and nothing less.

Take care.
