Suggest treatment to cure hypertension

Question: hi, i have been suffering from hypertension since 6 months.most of the time in a day my bp remains 140/90.my height is 6 feet and weight 115 kg.doctor advised me selomax 25 mg once a day.i have been through several tests to diagnose root cause of bp but nothing has come.i have become more aggressive & short tempered.one more thing ive noticed on average every alternate day or in 2,3 days i have excessive sweat, anxiety ,nervousness.then i have strong urge for food.when i eat anything spicy,sweet,or normal everything becomes normal immediately.i feel sleepy and exhausted all day . i also i have shortness of breath whole day and acidity.i am also taking pantoprazole 40 on empty stomach.i used to smoke and booze excessively but gave up entirely on both since last 6 months.please find attached reports.please advise.

Brief Answer: .... Detailed Answer: Hi there.... Thanks for writing in.... Your reports are all quite normal, except changes in ECHO, LVH... From your height , weight and BMI (body mass index), I understand that you are obese.. Once you lose some weight, your blood pressure might come under control.... Regular exercise 30 minutes a day 5 days a week might help. Get a personal trainer to help you out at the gym and make a weight loss plan. Reduce salt intake in your diet. Avoid pappad and pickle. Have plenty of fresh fruits and vegetables, salads with olive oil dressing, etc. Eat smaller frequent meals rather than 3 large meals a day. Drink 8-10 glasses of water a day. Avoid oily and fatty food, carbonated beverages etc. I really appreciate the fact that you have stopped smoking and alcohol since past 6 months. Keep it up .... My further queries are 1) Why do i start sweating for no reason and feel ok after eating food even if the food is spicy. 2) Doctor has advised me ABG test, is it important get tested for the same. 3) Why do i experience mood swings.

Brief Answer: ....... Detailed Answer: Hi... Sweating might be due to hunger,hypoglycemia (low blood sugar levels) or due to acidity related symptoms; which is why you might feel better after eating food. You did have high ketones in your urine test. That must be the reason doctor ordered an ABG test. Yes, please get it done. High levels of ketones in urine are seen in diabetes, prolonged fasting, excessive alcohol consumption etc. Mood swings are part of life.. However, if it is excessive or you are going in for depression, you might need medication. Do sign up for yoga classes which will help you relax... It will also help you lose weight. However, results are not immediate. However, in the long run ,it will be very beneficial for you.. Read motivational and inspiring books. Hope this helps.

My Further queries are 1) I get good 8-9 hours sleep every night but somehow i still feel tired and exhausted all day and i feel excessively sleepy during the day. 2)My stomach feels like it has gas all day, I've been taking PAN 40 for a while now but it is not helping.Please advise if i should take some other medicine for gas.

I would like to rule out OSA (obstructive sleep apnoea) , which is more common in overweight or obese individuals. OSA is a sleep disorder in which breathing repeatedly stops and starts during sleep.The most noticeable sign of obstructive sleep apnea is snoring. As a result of this, an individual might feel more tired during the day, even if he feels he slept for ample hours. Another possibility, is depression, might be making you feel fatigued and tired. For the gas and bloating sensation, a tablet of GELUSIL MPS might help. I would also advise you to visit a gastroenterologist who might do an endoscopy if necessary. Avoid foods that produce excessive gas like pulses, broccolli, cauliflower, potato, peas, beans etc. As I said earlier, regular exercise and yoga might help. Hope you feel better soon..
