Suggest Treatment For Severe Anxiety And Stomach Pain

Question: Hello Sir,

I suffered from gallstone in 2007. It was removed in 2008 after having laproscopic surgery. Afterwards my metabolism has been disordered, means to say whenever I eat junk food, my weight surges and also suffers from acidity. Due to acidity, I also suffers from halfside headache on frequent basis. Right now acidity is too much that I m doing burps and there is stomach pain also. I m a smoker (2-3 cigratte on daily basis) since seven years.

For acidity, generally I take Histac (from Ranbaxy) or Rantac 150 and for head-ache, take asprin or Vasograin (ergotamine, paracetamol, caffeine, and prochlorperazine)

Hi.
Thanks for your query and an elucidate history related to your gall bladder surgery, disturbed metabolism causing weight gain on junk food; acidity and headache problems and the medicines you take for this.

I would suggest you the following:
- First of all go for upper GI endoscopy so that we can find the the reasons and the effects for acidity/ulcer or so.
-Ultrasonography / CT scan of the abdomen again helps in finding the reasons .
- Thyroid functions, liver function and relevant blood tests,'
- CT scan /MRI of the brain with an opinion of a Neurologist to find the reason for headache and get an appropriate treatment.

In the meantime I would advise the following in such a patient:
Continue Ranitridine 12 hourly.
Pepcid every 3 to 4 hours when on empty stomach.
Add Domperidone or such motility regulators.
Soft bland diet.
No aerated colas / soda/ drinks.
Early dinner. No late nights.
Treatment for anxiety (as prescribed / needs a prescription by your Doctor)
Lying in a reclining position
Walk around after dinner.
Think of the factors which increased the present problem, GERD, anxiety and try to avoid.
If no relief, get Upper GI endoscopy done and get prescription for added Medicines.

FYI, I had done complete blood test and uploading the report for your review.

I have couple of questions here:

1) Continue Ranitridine 12 hourly - for how many days I have to eat this medicine?

2) Pepcid every 3 to 4 hours when on empty stomach - for how many days I have to take.

3) soft bland diet - What eatable things I should avoid.

Detailed Answer:
Take the medicines for 2 weeks to start with.
Once the symptoms are reducing reduce pepcid/ antacid to be taken only when on empty stomach.
Ranitidine to be continued as per the findings of the UGI Endoscopy.

Avoid: spices, anything with chillies, fried, junk foods and all those foods and beverages which you must have noticed that instigated or enhanced your problems.

Seen the reports almost within normal range barring a few that are just outside the normal range and the reports are of March 2015 , it looks.

Rest of the things as we already discussed.

Thanks a lot sir for your prompt reply.
I have started with the medicines told by you since two days.

I know that it will take time to cure all this but there is not at all any relief in acidity and was burping very frequently yesterday even after having very light food .

I am suffering from severe headache right now and everytime when i have headache it is a pain near the eyebrow region and i feel that it is totally related to gas and acidity because the pain subsides as the acidity relieves.

During headaches I was taking 2 disprins at a time and when it fails I took ALEVE tab(naproxen sodium 220 mg) till 20 days before. Sometimes none of the tablets work .I have analysed that if headache occures today near the left eyebrow region it definitely turns up on the right side the very next day ..thus completing the cycle..

From last 2 weeks, I was taking Vitamin C (1000 mg) tablet on alternate day, can it be the reason of acidity?

Pl guide me that how this acidity can be regulated. Waiting for your sincere reply.

Hi.
Thanks for your detailed feedback.

Aspirin, aleve and Vit c can cause very severe acidity.

I hope you are taking Domperidone 10 mg at 8 hourly interval.
It is possible that there is severe gastritis and takes time to settle.
You can add Omeprazole 40 mg once a day.
You may please ask your Doctor to prescribe something for the anxiety.

I can understand that the headache is related well to acidity, yet if it is so much severe it is very imperative to rule out any pathology by MRI of the head and sinuses.
And since you are not getting relief, it is better to undergo upper GI Endoscopy at the earliest to get the proper diagnosis.
