Suggest Treatment For Recurring Shingles

Question: My wife has recently had a recurring case of shingles on the left side of her face subsequent to cataract surgery and shingles vaccination. Yesterday she had a dull, one-sided head ache. Does she need gabapentin right away, or should she wait to talk to the doctor on Monday?

Hi and thanks for the query,

I do think a proper evaluation might be required for a better management of your wife. It s nice already she will be seeing the doctor.

Considering the fact that the shingles are recurrent, a long treatment with specific anti viral agents might be appropriate. Your doctor would evaluate if steroids could added or local treatment with local anesthetic agents like Lidocaine, depending on the intensity of the pain.

At this point, I think Gabapentin added to Tricyclic anti depressants (like Amitryptiline) could help.

A proper evaluation by your doctor would be useful. Checking for any causes of immune depression like a cancer somewhere, or at times even diabetes.

Kind regards as you prepare to see you Dr. Gabapentin and a Tricyclic antidepressant, with steroids or local Lidocaine 5 % could help.

have blurry vision. should I resume prednisolone eye drops?

Hi and thanks for the query,

I think its logical in my opinion to continue with the prednisolone eye drops. It is accepted and well known practice to take Valacyclovir one tablet, three times daily (after every eight hours) for seven days. My suggestion is as you rightly put it, to increase to one tablet daily, to three tablets daily, for 7 days.

A review from your doctor will check for the appropriateness of systemic steroids, Gabapentin, Tricyclic anti depressants and local 5% Lidocaine.
