Question: Pimples on vulva, blisters in the butt crack, rash on the cheek, on the right side. terrible sciatica preceding the rash, not sure if correlated. sciatica occurred the day after walking in heels vigorously (felt pain during the day after walking, got worsened).

Thanks for the query to HCM,

The pimples over vulva, blisters and rash along the butt crack are unrelated to sciatica as it seems. The most likely reason for the rash and pimples along the undergarment genital region seems to be due to contact dermatitis or allergy because of the material of the undergarment worn mostly. Ant polyester, silk or even latex , if not cotton can cause such allergic reaction which can be controlled with antihistamines like TAB CETIRIZINE 10 mg once daily at bed time.

Secondly , vigorous walking on heel for long if it initiated the sciatica pain then there is all likelihood that you have hurt your lower back and there may be a vertebral slip and nerve pinching causing the referred pain along the lower limb. You need to get a X-ray lumbo-sacral spine down to rule out and lumbo-sacral listhesis or slip disc.

You may visit your PCP for a antihistamine and a pain killer prescription, and a X-ray spine is mandatory. However you may send the pictures of the rash , pimple lesions as follow up to rule out other possible differentials like an STD, If you have been sexually active recently.

Hello,
I have reviewed your attachment and gone through the pictures. I am afraid but these are classical herpes zoster with clear fluid blisters and confluent growth on one side of the spine alone a particular dematomal distribution, in your case S3-S4 region.
These are not cellulitis or allergic/ dermatitis rash for sure and neither impetigo, but can be painful and with red inflamed base, marks the start of herpes zoster. One can wait for the results to confirm the same. You should be started on antivirals soon to reduce the length of incubation period and the effects.

Thank you Dr Asmeet. Can you get shingles although you had chicken pox vaccination as a kid? My mom thinks I had the vaccine... I was experiencing some pain on the right side of my torso and poking sensation on my abdomen sometimes, so I started on the antivirals that my NP prescribed just in case.

Yes, even if you have been vaccinated with chicken pox vaccine still there may occur reactivation of the varicella zoster virus , residing in the dorsal nerve root ganglion , especially when the general immunity of the body is low. However as seen,15-20% of children vaccinated with 1 dose remain at risk for varicella due to lack of immune response or partial protection. So this may be a possibility in your case.
