Question: Irregular period

I am 27 years old unmarried female. I have problem of irregular period. menses started at the age of 12 years. since then it is irregular. Few medicine tried but not effective.
earlier there was no issue of any type of pain either bone or stomach ache, but later stomach ache have been regular in each period, may it be first day or the next or sometimes throughout the period. Gap between the periods is alwayz more than 40-45 days or sometime even more than that. In short its completely unpredictable the day of period. heavy fall and more clotting is normal.
presently, In last few months what i realised is the stomach ache is less and the back pain is more sometime unbearable during the period. short of nausea and heaviness in body.
period last for 5 days, but the gap betwen the period is same i.e more than the 40 days.
Continues white discharge is another problem. Kindly suggest suitable and effective treatment.

Brief Answer:
Poly Cystic Ovary should be ruled out.

Detailed Answer:
Hello Dear,

Thanks for writing to us. Followings are my comments:

1) Some underlying causes of irregular period are Poly Cystic Ovary, mental stress, emotional upset, hormonal imbalance, over weight or sudden loss, irregular diet pattern, pelvic infection, high prolactin etc.

I suggest to undergo some basic investigation to narrow down the causes and confirm the diagnosis.

2) I suggest to undergo one pelvic ultrasound scan and complete hormone profile (like Serum estrogen, progesterone, LH/ FSH, serum TSH, Prolactin, Testosterone, DHEAS etc) to rule out underlying pathology. You can consult with your nearest OBGYN for proper guidance.

3) Combined hormonal pills are the mainstay of treatment to regularize your cycle. It can be prescribed after reviewing test reports.

Take healthy diet with LOW carbohydrate and IRON supplements, do light to moderate exercise for physical fitness and to control body weight, avoid mental stress completely by regular meditation or yoga. Maintain a healthy life style always.

Hope, I have answered your query. Wish your good health and take care.

The cause of irregular periods which might apply to me probably are emotional upset and irregular diet pattern. There may be other cause also for which i'll go for investigation as suggested soon. Till that time please suggest treatment according to all possible causes of irregular periods. Kindly prescribed Combined hormonal pills, if it is safe. I am not over weight.

Brief Answer:
Treatment plan is given.

Detailed Answer:
Hello,

Welcome back again.

Treatment of irregular period depends on underlying cause and you need to undergo above mentioned investigations to confirm the diagnosis. It will help to determine right medicine or combined hormonal pill.

General treatment guideline:

1) Treating underlying disease like poly cystic ovary, anemia, thyroid disorder etc.

2) Using combined hormonal pill like FEMILON etc ( 3 weeks ON & 1 week OFF) from D1 of cycle.

3) Life style changes including intake of regular small meals (3-4) in right time. LOW carbohydrate is preferable.

4) Avoid mental stress or emotional upset by regular meditation or yoga. Do regular 30 min walking for physical fitness.

5) Surgical treatment like Laparoscopy may be needed if multiple cysts are detected on ultrasound and failure of hormone therapy.

Hope, it helps for your information. Good luck and take care.

Brief Answer:
Hormonal pill will regularize cycle and reduce period cramp.

Detailed Answer:
Hello dear,

Welcome back again. Followings are my comments:

Menstrual cramp (lower abdominal pain) or painful period is called as dysmenorrhea. Combined hormonal pill (like FEMILON etc) will be much effective to regularize your cycle and painful period.

However, you should undergo above investigation before taking any kind of medications. Some supportive treatment may be required.

For back pain, you should use muscle relaxant gel and hot water bottle beside above treatment. Avoid fatty food, tea, coffee, cold drinks, ice cream, salty item in the 2nd half of cycle. Kindly, consult with gynecologist for complete prescription.

Hope, it helps for your information. Good luck and take care.
