Question: Hello Dr. Rynne,
My 7 year old son has a fever of 102 since Christmas Eve. I have been implementing cooling measures i.e.. tylenol/motrin and tepid shores. He vomited this afternoon and has been complaining of Left Lower side pain since then. 12p. It is painful when I push down into the affected area. Should I be more concerned then treating him for flu like symptoms?

Hi...Thank you for consulting in Health Care magic.

Fever of 4 days without any localizing signs could as well a viral illness. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require antibiotics at all. Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.

Pain in the left lower region could be due to either constipation or colitis. Unless the kid's having low urine output or very dull or excessively sleepy or blood in motion or green bilious vomiting...you need not worry.

Paracetamol can be given in the dose of 15mg/kg/dose (Maximum ceiling dose 500mg per dose) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

Give him plenty of oral fluids.
