Question: I have hives all over my body. I have an ileostomy. My stoma is swollen but not purple or black just really dark red.. I am passing stool oK. However, I do have a temperature of 99.6 and a very swollen belly. I had rashes before many times over the last year but have not felt this ill. I saw a dermoltoligest who put me on zertec. I had been taken benderyl for the last week or so with no reesults. I am seeing an allergest after Zertec treatmet. I really am swelling up especially in my abdominal region. What might I have and what should i be doing that I am noyt doing already?

Brief Answer:
Please go in to ER.

Detailed Answer:
Hello XXXXXXX

For chronic hives, higher doses of Zyrtec can be used than the standard dose for nasal allergies. However it sounds like more is going on with you than hives only, given that you have a low grade fever and abdominal swelling. Without being able to examine you, I do not want to recommend anything. You need to go in to an ER to be examined tonight if your abdomen is swelling. I understand that you have an ileostomy. It's possible that there is a blockage or an infection happening. In the ER they can examine your abdomen, get a complete blood count, and get imaging studies (starting with X-ray) to see what is going on with your abdomen. This is concerning, and not something we can diagnose from description only. So please do go in. I welcome you to let me know how you are doing after you have been evaluated.

I should have told you that I have been in the ER several times this year. They did x-rays and cAT scans. They only found a papastomal hernia. Blood work was OK also I just went to the eR in August of this year. all was oK except the hernia. surgeons will not do surgery, not even specialized hernia specialists because I have had 9 abdominal major surgeries over the past 20 years. I spoke to 5 surgeons over the last yeat. I have told them repeatly thAT i HAVE A VERY SWOLLEN BELLEY AND i AM VERY sore. However, last evening, my swelling was worse than usual. I do have repeated bouts of low salt ensemes and some minor issues with creaton levels being one or two points high or low. I took sodium bicarbonate to be sure my enzyems get adjusted and I took the zertec. I feel somewhat better but still have some swelling. Dr. Berger-Dembaugh: I am sick of having doctors say I do not have something going on. I really believe I have some kind of infection that they cannot pinpoint. I believe a needle biposy would be the best means of dedtecting an infection. several years ago I was in the same position. Not even repeted CAT scans, bloodwork, repeated stool checks, and other tests could detect an infection or if I had sepsis and an absess. The needle biopsy detected that I had three really serious infections and sepsis. They did the needle biopsy was taken on the right side of my belly and showed that I had sepsis, a severe candidia infection, that needed emergency surgery, and VRE. I almost died of these serious infections. I also had an absess going on. My white and red blood cell count and hemogloben tend to be low. I don't think in my case that blood work or urine analysis stool checks, etc. actually are good indicators as to what is going on with me. I even went to the XXXXXXX Clinic this year. They did a fibroscan and several other tests and they only found a slight elevation in my liver, indicating a slightly non alcholic fattty liver disease, a minor scraping on my small intestesine. a parastomal hernia, a slightly enlarged spleen, and two small cysts on my kindey that they are not concerned about. My pain is not in my liver area. It is in my intestional area. I am extremely frustrated that the doctors will not listen to me. A regular liver biospy not just a fibroscan, in my opinion, may detect that I have an infection. I rarely run a temperature. My normal tempature is 97.6. I am so damn frustrated and afraid that I will wind up back in the hospital needing emergency surgery for an infection or hernia strangulation. Abount 4 days a week i am in so much pain in my abdominal area that I have to stay in bed. The doctors think it is only my hernia that is causing my problems. From what I understand hernia sacs can become infected too. I am sorry that i am dumping on you but I am at my witts end with this problem. I would like to know what you think. Could I be right about the needle biospy for infection detection? Or what else can I tell the doctors about my condition? I am about 60 lbs over weight. I am trying to work on a gluclose free diet. I still believe I have some kind of infection going on, and no matter what I do to fix it on my own, i cannot heal the infection if there is one without treatment. Some doctors have also said that i may have a virus. they have tested me for hepititus. All these tests were negative too. I also have bipolar illness. Again, I am very sorry that i am dumping on you. Please refer to my post concernng Dr Chandrakant for his opinions. I did what he said for me to do, too, no luck. XXXX

Detailed Answer:
Hello XXXXXXX

You do not need to apologize for detailing your history. But I am thinking at this point that it is beyond the scope of my knowledge as a family practice doctor to be able to answer. I am thinking either a general surgeon or a gastroenterologist would be able to sort this out better than I can. I suggest reposting the history you provided here, and at the beginning of it write "For gastroenterologist only". You can also post the new question with "For general surgeon only", too.

So
1. Write who you want to answer your question at the beginning.
2. Next write a specific question such as "Given the following history, what could be the cause of my abdominal swelling and pain, and low grade fever?"
3. Post all the rest of the history you have here.

I hope this helps, XXXXXXX I am really sorry for all the trouble you have had.
Note: Hope the answers resolves your concerns, however for further guidance of skin related queries consult our Dermatologist.Click here to book a consultation
