Question: hi .... i have uncircumsied penis .....i had sex with hooker or sex prostitute ......she gave me oral with condom then after some time we had missionary posistion with same condom then i ejaculated but with condom and she removed with tissue paper and wipe it properly then after some time i fingered her vagina then she gave me oral with new condom and then she changed the condom then she was on my top we had intercource but i ejaculated inside her and when i was taking my penis from her vagina condom was inside ....i am scared i asked she said she never have intercourse without condom ....please tell me what are my chances of having hiv ..i had protection all the time ... she also kissed my lips and my chest is that ok no passionate kissing ...supose imagine if she is hiv positive girl and i am hiv negative what are the chances i have heard female to male transmissiion of hiv is less . what should i do this incident happened 2 days back . i have next day blurr vision like diziness but its due to my oral steroids i am using for gyming i dont know . THERE is no pain when i piss and no boils on my penis .suppose if i have eye infection conjunctivirus after this incident means i have hiv . please help . one more thing what is the duration for hiv test i do hiv rna pcr or wait for 28 days . tell me the test name . please guide me i am in stress now

I have read all your question and narration as said by you and will give you the answer.

First of all, I would like to tell you that HIV infection occurs only when our mucosa linings in the mouth, vagina, penis or rectum come in direct contact with blood or bodily fluids such as semen, vaginal fluid and rectal fluid that can contain HIV virus.

Also,you had sex to that girl with 'condom on' most of the time and only it might get slipped inside her vagina when she was on top of you.

1) Therefore in your case, if there was no cut or scratch or even bruise on your penile skin then,I do not think there is any chance of getting HIV in this scenario.

2) Through bare hands or bare body skin or even by just lip to lip kissing (excluding deep mouth kissing), the virus cannot get transmitted into other's blood circulation.

Also, the materials used to make most condoms such as latex, polyurethane and polyisoprene do not allow bacteria, viruses or other germs to pass through them.

Therefore condoms act as a barrier to HIV infection by preventing the mouth, vagina, penis and rectum from being exposed to bodily fluids such as semen, vaginal fluid and rectal fluid that can contain HIV.

3) Yes,chances of female to male transmission is less (like around 2 to 3 %) and it is only when there is HIV positive female and you had a forceful unprotected sex with tearing of your genitals during sex which has not occurred in your case.

Therefore, do not bother and be rested.

4) Your eye infection (conjunctivitis) could be due to some other infection (other than HIV) for which you can take appropriate treatment from some local doctor or GP (if you still have the symptoms).

5) PCR-RNA qualitative test for HIV is highly sensitive & specific test, you can be sure of the test result (as a negative) after 25 days of exposure (90% accurate) since the chances of false negative result will be rare here.

Generally incubation period of HIV ranges from 1 month to 3 months and if you did the test after 25 days of exposure then it could give around 90 to 95 % accurate result.

So,you can go for this tests after 25 days of exposure to clear remaining doubt but the chances of HIV negative will be greatly.

OK i am not cleared from my question ...please guide me again ....i am uncircumsized is that dangerous ..like i mention earlier condom slipped when i was removing my penis out frm vagina ....and imagine that girl is hiv positive so with first encounter with one sexual encounter i can get hiv ......i want to ask that ....ratio means like out of 1000 only 1 can be infected its like that ....please explain in detail ....this incident happened on 8 nov 2015 ....tell me i have read that it takes 20 to 30 times to get hiv from hiv positive person ..my incident i was protected but condom slipped ....please explain in detail ......is pep is allowed in XXXXXXX ..can i go on wed for pep programme and ask for doctor ....incident happened on sunday and one more thing u said window period for rna pcr is 25 days ..but pathology services said window period is for 14 days ...i should go for rna pcr or wait for 28 days and hiv duo ....please guide in detail .......in future i wanna ask more question how to reach u ..sir waiting for your reply ..please clear all my query ......which i mentioned now ........thanks doctor

Thanks for reply.

1) Uncircumcised penis is not so dangerous or high risk condition for HIV transmission.Means circumcision of penis does not provide full protection against HIV.

2) With first sex encounter,it is very rarely to get HIV even if the girl is HIV positive especially when there is no cut or scratch or even bruise on your penile skin.
Means through bare penis skin (when come in contact with vagina or its fluid) or even bare body skin HIV transmission is not possible.

3) Suppose if that girl is HIV positive and one will do sexual intercourse forcefully (with tearing of penile tissue and vaginal tissue and blood leaking) then it does not necessarily mean that you will get infected with only first time.It will require multiple sex encounters like you said 20 to 30 times.

4) pap test (Papanicolaou test or cervical smear test) is used to detect pre-cancerous and cancerous cells in cervix of a woman.
It is not for HIV.There is no need of pap test in your case.

5) Window period for PCR RNA test is approximately 2 to 3 weeks but it gives results with only 90% accuracy.If you do that test after 25 days then it may give you nearly 100% result.

Finally,in your case there is no chance of contracting HIV from our all discussion but still if you want to clear any doubt then you may go for PCR RNA tests after 25 days of exposure which will give you final confirmation.

hi ..doc happy diwali .....can u please tell me today i checked my penis its uncircumsized but saw red colour mark dont knw it was before or not ..but i hve normal boils on my penis forskin ..before i had sex .....please explain detail .........yesterday on my forhand there were rash but today it disappeared .....and on my neck back side there are red rash ...but on my chest red rash from long time .......one more when i had sex three days back which i explained u ....next day i was feeling chill types like i switched off my fan feeling cold ....i stay in XXXXXXX .......please tell me if u dont have sore throat and fever and pain on your penis but u have rash it means hiv is started .....hiv virus can be happen after 24 hours or it takes 2 weeks minimum ......please guide me what test should i take now .....tell me all the std test i should go with window period so i can go for test coz after 3 months i am getting married ......my penis does not pain when i pee or in normal case .......but that boils on my testicles and penis making me scared but i cannot see many ..when i touch there i dont get pain ..please guide ......in future when i will do my test i will be in touch with explain my query in simple way ......

I have read your query in detail.

First,kindly send me the picture of your glans penis to have a single look at those red colour marks, I will give my opinion after inspecting the lesion.

As you have developed rash at some part of body after 2nd or 3rd day of exposure, but we can not think of HIV from these rashes because it might take up to 2 to 3 weeks to develop symptoms like flu or rash. Feeling cold or chills could not be the symptom of early HIV.

Means it is almost impossible to have HIV symptoms within 24 hour.So do not worry about it.

If you want to know about all STDs then you can visit nearest VCTC center.

For,gonorrhea the window period is 7 to 10 days,for syphillis it is 15 to 20 days and for HIV it is 1 month to 3 months (maximum upto 6 months in some cases).

So,at present I do not think you should go for any tests for STDs.

Gonorrhea and syphillis are quite uncommon nowadays but for HIV,you can go for either RNA qualitative tests or ora quick test kit at home but only after 25 days exposure to rule out any false negative results and to clear remaining doubt.

It is good that you are getting married within few months,but there is no need to be worried unnecessarily.

As far as I am thinking,you won't have any STD (especially HIV) from our discussion.
