Suggest Treatment For PCOS And Cyst On Ovaries

Question: I've had several urine tests and at least one blood test. I've had one general ultrasound and all are negative for pregnancy. I've gained 7 pounds each month for the last 3 months. My belly is tight and larger. Hormones seem out of whack with pregnancy symptoms coming and going. My doctor thinks it's all in my head. I have pcos and cyst on ovary. Painful sex with seemingly swollen vaginal walls with sex. How can I either confirm what I feel to get treatment or should I accept what I am being told? I've never been pregnant to my knowledge. My hands and feet are swollen. I'm exhausted and appetite increases at times and I get headaches more lately. Bleeding hasn't stopped montly but is weird. Sometimes scanty or may flow one day then spot. Very irritable and depressed at times. I swear my pulse in hand is stronger and even my husband felt it when holding my hand. Upper belly round and tight also constipated daily. Period like cramps when not on and at night it seems that slight movement is present. Can't seem to get any doctor to dig a little deeper.

i read your question and i understand your concern. Polycystic ovaries syndrome can be bothersome and can be a very common cause of primary or secondary infertility. The symptoms like pregnancy are due to hormonal imbalance common in PCOS. Periods irregularities, weight gain, hirsuitism ( excess hair growth on face and body parts) , painful coitus, bowel symptoms like constipation altered with irritable bowel etc. You need complete work up to find out any cause for PCOS. Work up for cushings (cortisols, 24 hours urinary cortisol, dexamethasone suppression tests), thyroid ( Thyroid function tests to rule out hypothyroidism ) etc all need to be done to rule out any possible metabolic cause for this PCOS.

Treatment aims at three main things. Losing weight and keeping BMI to less than 25, regularising menstrual cycle which will for sure once weight gain is tackled and use of oral contraceptives is added to it . Last aim is to try fertility, use of selective estrogen receptor modulators are used like clomiphene to induce ovulation which is a real challenge unless first two aims are achieved. It may be lengthy process but eventually there is hope. Meanwhile, modifying diet and lifestyle is mandatory, DASH diet, ADA diet, small frequent meals, reducing caloric intake, more exercise and consumption of calories is advised.
