Question: Hi

I am 21 years old. I am alcoholic from 7 months 6 to 7 times a month not a regular smoker . I met a gastroentrologist 2 months ago due to slight pain to slight right just between the rib and abdomen consistent for 2 days. Few tests are done and he said nothing is wrong. And he said to take 6 small meals.

For that 2 weeks am soo worried and tensed about my situation.

I got lower back pain for 3 to 4 days in that 2 weeks. Back pain Stooped

Eventually that pain on the slight right stopped no sign of it.

I stopped eating oil foods and Nov veg items and alcohol also.

Here strats my problem after that my upper abdominal area(frequently) started paining like 3 on 10 scale sometimes 2 on 10 scale.pain in not consistent in one area some burning sensation stays for 3 to 4 seconds goes away. But some discomfort in abdominal area present Some times around navel region(not frequently).

Before this situation my diet at nights is only milk . I went to gym for 3 months almost same diet may be after 1 month I observed that 10 percent of stool is loose and 90 percent is solid

After I left the gym but sometimes I observe that 10 percent or 15 percent of of stool is semi solid may be 3 times a week

Abdominal burning is still present not all day long but I feel it after eating. Less food doesn't cause any much pain. If I eat more I get heartburn too and abdominal pain not pain actually some feeling of burning is common . Blecthing and gas increased after that 2 weeks

I am worried of my situation. Please suggest something

Tests before 2 months:

Upper endoscopy -- no significant findings

Abdominal ultrasound sound-- pancreas is mildly small(what does this mean)

CT Scan: No significant findings

Serum CRP: 2.5 mg/L

Serum Amylaze:66 U/L

Serum Lopez : 120 U/L

Random blood sugar :97

Brief Answer:
GERD/IBS, following advice...

Detailed Answer:
Hi XXXX,

I have gone through your medical history carefully and understood your concerns.

1. You are saying to having this abdominal pain that sometimes is felt like burning sensation associated with belching.

All these symptoms are indicative for possible GERD or IBS syndrome taking into account that all your examinations are fine.

2. I advise to:
- eat often and small portions
- avoid spicy/chilly/oily/fried foods
- avoid laying down immediately after eating
- avoid foods that would cause gas
- take PPIs in the morning and antacids after meal

3. With regards to the small pancreas noted, that is not a problem but it is a variant of the normal size. I need to have a look at the abdominal ultrasound myself before giving the final conclusions. If possible please upload the scan report on this page.

Hope it was of help!

Hello doctor ,
Thanks for answering and going though my profile with patience
I think I missed some details
1)I don't have constipation or diarrhea. I have one bowel moment a day. In a week may be 3 times my stools contains 15 percent semi solid (any serious issues with it?)
2)before the onset of the pain I used drink milk only at night times. For may be 1 or 2 months , 15 percent of my stool is loose
3) my heart burn not frequent. If I eat a bit heavy it burns on chest
4) my pain is not localized and. and not radiating. Its pain or burning(50 pain 50 burning)am not able to decide. Most of time burning is on upper abdomen sometimes around navel region
And very less frequent burning on upper lower back for 5 seconds
5) In CECT mild hepatomegaly is mentioned(please check the attachment)
6) any problems with my pancreas ?
7) do I have functional dyspepsia ?

Detailed Answer:
Hi back XXXX,

1. There is no serious problem with your semi-solid stool. It means that you need to increase water intake, fiber, and physical activity to help/ease your bowel movements.

2. You used to drink a cup of milk every night without any problem. Only that night you had stomach pain. This is not suggestive of lactose intolerance as long as you have only episode of pain after drinking milk. However, to make sure no lactose intolerance is present should run the intolerance testing.

3. Feeling heartburn only when eating heavily and fatty foods are suggestive for GERD (acid reflux) problems.

4. The burning sensation you are feeling is also another sign for GERD.

5. The mild hepatomegali is possible medications you might be using recently.

6. In the ct-scan report, there is no problem with your pancreas. The small size it was found during ultrasound is showing only a variant of normal size.

7. Your symptoms are indicative for GERD, and not functional dyspepsia.
