Is Surgery Advisable For The Presence Of Soft Tissue In The Sinuses?

default
Posted on Thu, 30 Mar 2017
Question: I have soft-tissues in both the left and right maxillary sinuses. The CTBT report says that the left maxillary sinus is almost filled with soft tissues and the right maxillary sinus is filled less than 50% with soft tissues. Is it must that I go through a surgery ?

Brief Answer:
maxillary sinus mass.

Detailed Answer:
Hi thanks for your query. In my opinion, you might be having bilateral chronic maxillary sinusitis causing maxillary sinus polyp. Yes surgery is the answer for this. Surgery is called Fess. but empirically you can try steroid nasal spray 2 puff twice daily for 6 to 8 weeks + steroids tablets methyl prednisone or prednisone Short course for 2 weeks and see. All these are prescription medicines, so I would advise you to consult with the doctor and get evaluated properly. Hope my answers have helped you. Thanks.

Brief Answer:
fess is the answer

Detailed Answer:
Hi, thanks for your query. You need to try medicines only for 2 weeks to 3 months maximum. If the polyp has not shrunk. Then surgery is the answer . Polyp and soft tissue both are more or less same for a laymen. Only etiology and histopathology a nd treatment varies. But for polyp medicine is the first line of treatment. For any other soft tissue. Surgery and biopsy is the first line of treatment. Hope my answers have helped you. Thanks.

Greetings Dr. Deepashree,

How many percentage of people do generally have polyp in their Sinuses and how many percentage of people do generally have other soft tissues (other than Polyp) in their Sinuses ?

Can Homeopathy cure soft-tissue, irrespective of whether it's Polyp or other soft-tissue, build up in people's Maxillary Sinuses ?

Thanks,

Brief Answer:
Not to worry.

Detailed Answer:
Hi, thanks for your query. San XXXXXXX as per ct report, Your left maxillary sinus has just mucosal thickening . This is usual seen in most of patients. It is asymptomatic and does not need treatment. Your right maxillary sinus is completely hazy. Does not show any soft tissue or polyp. But suggest maxillary sinusitis. His can be treated first line steroid spray, antibiotics and anti histamines for 3 weeks and then repeat scan if the haziness persists then fess surgery is needed. But all this is only if your sinusitis is symptomatic, like headache if, running nose, sneezing etc. If not no treatment is needed. Hope my answers have helped you. Thanks.

Greetings Dr. Deepashree,

I am not 100% asymptomatic.

1. I have "slight" loss of smell. I don't get small smells but I do get moderate & big smells.

2. I have a slight nasal blockage just alike when you will have very light cold but I don't have any cold. This is light or very light.

But I DON'T have running nose, headache, sneezing and others.

Under these symptoms, is your last writing above still valid ?

Thanks,

Brief Answer:
yes valid.

Detailed Answer:
Hi, thanks for your query. Yes sir. Slight loss of sense of smell with nose block are the features of chronic sinusitis. Detailed endoscopic examination of nose will help the ent specialist to evaluate whether pt can be treated medically or surgically. I would suggest you to consult with the doctor and get evaluated. Hope my answers have helped you. Thanks.

Greetings Dr. Deepashree,

I am so sorry to get confused, which would you please clear ?

In the 2nd last answer, you said ....."But all this is only if your sinusitis is symptomatic, like headache if, running nose, sneezing etc. If not no treatment is needed.".....

And in your last answer, you said ....."Detailed endoscopic examination of nose will help the ent specialist to evaluate whether pt can be treated medically or surgically.".....

Since I am not 100% asymptomatic, is this why you said if I can be treated medically or surgically, instead of "no treatment is needed" ?

I am so sorry at first if I am making you feel contradictory, for which I am apologetic in the beginning. But I am so scared of surgery that I am asking you this question.

Sorry again.

Brief Answer:
consult with the doctor.

Detailed Answer:
Hi,
thanks for your query.

There is nothing to be confused. As per your ct you seemed to have sinusitis . Sinusitis is treated first with medicines then If it can't be treated then it is treated surgically. Since u don't have any symptoms of sinusitis,But your ct is showing the features of sinusitis , we need to evaluate whether we should treat you or not. This can be done by endoscopic examination of nose and sinuses. Then decide to treat you more so with medicines only. Soft tissue or polyp is not there in sinuses so don't worry.

I hope i am clear. I would suggest u to consult with doctor and get evaluated. No doctor will operate unless there is a indication to operate.

Hope my answers have helped you. Thanks.

Greetings Dr. Deepashree,

Thank you.

Sorry, but in your 2nd last answer, you said ....."Slight loss of sense of smell with nose block are the features of chronic sinusitis."..... and in the last answer you said ....."Since u don't have any symptoms of sinusitis,But your ct is showing the features of sinusitis"..... Which is correct, whether I have symptoms of Sinusitis or not ?

Also since you said ....."Soft tissue or polyp is not there in sinuses so don't worry."..... Should I assume that I won't have to go through surgery, correct ?

Thanks,
XXXXXXX

PS : Can Homeopathy treat Sinus issues I am having ?

Brief Answer:
consult with the doctor.

Detailed Answer:
Hi, thanks for your query. Yes u don't have polyp or soft tissue so u don't have to undergo surgery for that. But for chronic sinusitis not responding to medical treatment u may need surgery . That can be decided after clinical evaluation. I have no idea about homeopathic treatment. U can try it if u want. Yes u have mild symptoms of chronic sinusitis , like loss of sense of smell and nose block. This can be treated medically . Hope I am clear. Thanks.

Greetings Dr. Deepashree,

On 21st January, 2017, I showed to an ENT specialist, when I hadn't done the CT scan and didn't even know that I have something in both my Maxillary Sinuses, for this loss of smell and he did a nasal endoscopy and it's written on the prescription that something is N/A - here is the prescription :

http://www.mediafire.com/file/oep3uv6c17jt824/Dr.+Kaushik-Roy-Prescription.pdf

The nasal endoscopy is written on the top-right corner of the above prescription. I also took the following medicines for 2 weeks - ***the nasal smell ability improved*** but I caught cold and couldn't understand the effects of the medicines. But later the same loss of smell.

The followings are medicines I took with their compositions :

1. Name : Momeflo Nasal Spray [Composition : Mometasone Topical (50mcg)]
2. Name : Relent
3. Name : Omnacortil 20 & Omnacortil 10 [Composition : Prednisolone (20mg) & Prednisolone (10mg)]
4. Name : G-Cin 320 [Composition : Gemifloxacin (320mg)]

Please tell me what you think about the nasal endoscopy he did and of the medicines I took ?

Thanks,

Brief Answer:
Correct treatment.

Detailed Answer:
Hi, thanks for your query. As per endo scope report there is nothing major to be concerned. Treatment given is also correct. I feel u may have to continue the nasal spray for 6 weeks to 3 months with anti histamine tablets . After 3 months repeat the ct scan and see. If the sinusitis features are persisting then u may need surgery.
