Is Pregnancy Possible After Taking Unwanted 72 Within 48 Hours?

Question: Dear doctor, i had unprotected sex on 23 rd nov. And took unwanted 72 within 2 hours. I had early periods/ implantation bleeding on 7th nov. It started with brown spotting for two days followed by bleeding blood red fir 3 days and brown spotting again on 6th day. My lmp was 15 nov, so was expecting my periods on 12-15 dec. I took a beta hcg and serum prolactin test on 13th dec. ( values <0.100, hcg and 46.6 prolactin). Can i be pregnant? The test was done when i was still spotting. Was the test taken too early?

Iam Dr.Srilatha gynaecologist.I have gone through your query.
In my opinion the bleeding appears to be mostly due to period.
Because the implantation bleeding will be lighter in amount and does not prolong for many days.
Moreover the HCG level is very low.Usually HCG level < 5 mIU /ml rules out pregnancy.
The prolactin level slightly appears more than normal.Usually the rise in prolactin is caused by stress,timing of the test,any drug intake increasing prolactin level etc.
In your case stress might be the cause.
The ipill is 95% effective if taken within 72 hours of unprotected sex.As you have taken within time you need not worry.
The ipill causes hormonal imbalance leading to effect on timing and bleeding of the period.
So the early onset of period along with irregular bleeding might due to hormonal imbalance created by ipill.
The chances of pregnancy are rare.
So relax.
I suggest you tp repeat the prolactin level after a week or 10 days between 8 am to 10 am because time of the day might also effect the prolactin levels.

Thankyou doctor, my bleeding seemed lighter than regular periods, also there was hardly any cramps or pain with the bleeding. Its the 2nd day after the bleeding has stopped i am having frequent urination and slight pain on left side of lower abdomen and lower back. Have i taken the pregnancy test too early ? Or is pregnancy completely ruled out. And i was diaognosed with pcod 2 months ago, my prolactin levels were 25 at that time. Can increase in prolactin be a because of possible pregnancy?

I would like to explain some important points:-
1)Implantation bleeding-It will be hardly one drop like spotting.Anything more than this is not implantation bleeding.
And the lighter bleeding is due to period and this rules out the pregnancy completely.
Your HCG level is also strongly correlating with the above statement that chances of pregnancy are nil.
2)Polycystic ovarian disease(PCOD):-Many patients with PCOD will have difficulty in becoming pregnant without any medical support.Raise of prolactin levels is because of PCOD.
And prolactin levels are not standard diagnostic test for pregnancy.And periods in PCOD are not painful as the cycles are anovulatory.
3)Possible urinary tract infection(UTI):- Increased urinary frequency is seen in early pregnancy.But it is a very vague symptom and most commonly it is seen in UTI than in

Thankyou so much doctor, for assuring me that there are no chances of pregnancy. Can lower back pain and left side pain in lower abdomen can be a cause of UTI? Or maybe thats psychological, as i was too stressed regarding the pregnancy thing ? Also, i guess there is loss of apetite , coz i hardly eat/ feel like eating anything. may be due to stress. Or are these symptoms of UTI ? Should i get another beta hcg test and prolactin test done to reassure myself ?

Dear doctor, i got my prolactin and beta hcg test done today. Prolactin is 18 and beta hcg is 0.122 . (Hcg after 3days of previous test and prolactin after 1 week of previous test). Also i was taking B long F and nopicos obes twice a day from last 3 days. My doctor said she cannot give me hormones for pcod as they will affect and jncrease the size of my fibroid. Has the prolactin reduced because of these supplements or reduction in stress? Also y has my hcg level increased from <0.100 to .122 ?? Does this mean that i am pregnant? Or can it be ectopic pregnancy ? Thats y the hcg levels are low.

Hi dear,Iam very happy to answer you again and also it is nice to see that your prolactin levels are stabilizing.
The decrease prolactin level can be attributed to reduction of stress.
I strongly agree with your doctor that fibroid might increase size due to hormonal therapy.
So it is better to depend on non pharmacological methods like diet ,exercise and stress control.
Coming to your HCG report the change observed is very negligible.Even in ectopic pregnancy the HCG will be more than 5 mIU/ml.So normal pregnancy and ectopic pregnancy both are ruled out.
Weight reduction is first and most important on PCOD.So concentrate on weight reduction by adapting a healthy lifestyle.
Exercise also increases a kind of hormone called endorphin which helps in reducing the stress and feel confident.
Iam repeating one more time that chances of pregnancy and ectopic pregnancy are nil.

Dear docotr, i am still really tensed regarding the pregnancy thing. I read it on internet about a lady who got negative pregnancy tests for two months, but was still pregnant. Is this possible. Did i take the test too early? Or in my case pregnancy is completly ruled out? Or should i test again and be sure only once i get my periods for next month? Please help me with this.

Yes you are right there is a chance of having pregnancy with negative urine pregnancy test which might come positive after one or two months which is seen in rare cases.

Thankyou doctor, how early can serum bhcg can be taken and be relied upon? I hope the test taken wasnt too early. Also, as suggeseted by you, i took the uti test and the reports were normal. The reason i'm mostly thinking about possible pregnancy is frequent urination. I have to go to bathroom in every 2-3 hours. Can it be because of weather? As its cold here ?

Iam very happy to see that urine test for infection is negative.
Usually BHCG should be above 5 mIU/ml 3 weeks from the last menstrual period.
And as per the literature BHcg above 25 mIU /ml is considered positive for pregnancy.
Also the most important point is whatever may be the initial value BHCG will double in 48 hours which did not happen in your scenario.
It is quite common to have frequent urination in winter months because loss of water in the form is sweat is very less and water loss will be there in the form of urine.
Also as your urine examination is normal nothing to worry.Relax.

Thanks . Thats a relief ! In my case.. I had negative bhcg at around 28 days from last mestural period. Which completely rules out pregnancy in my case ? I guess i'm thinking too much and should just concentrate on stop thinking about this . Thanks doctor for being a great support and help. :)
