Is It Normal To Have Heavy Periods After Medical Abortion?

Question: My wife had a medical abortion 7 weeks ago. She bled for 2 weeks then stopped for two weeks. She had her follow up ultra sound and they said everything was fine. Her period came about 5 weeks after. It was very heavy with a lot of clots.We called the clinic and she was put on birth control. It stopped the heavy bleeding, but she is still bleeding 5 days after taking the birth control. She has had her "period" for about 13 days now. Is this normal? We have a follow up with a obgyn this Thursday. The nurse at the clinic keeps saying this is normal.

I have analyzed your query, heavy bleeding after abortion could be due to: 1) retained products which might have retained in the uterus , but this is already ruled out with the scan showing normal report, 2)Infection, this is also not a possibility (because I presume your wife is not having any fever or foul smelling vaginal discharge or pain lower abdomen) 3)hormonal imbalance, this is what I feel might be the reason for heavy bleeding. I agree with your doctor for starting birth pills. I usually put my patients on tranexamic acid + mefenamic acid combination drugs for controlling heavy periods. kindly speak with your doctor for a prescription. Usually your wife's period will become alright with birth pills, and I usually put my patients for minimum 3 to 6 months of combined oral contraceptive pills.... I also advise you regarding other contraceptive methods which you can choose from like copper-T, Levonorgesteral Intrauterine devise or permanent sterilization (if you have completed the family). Thank You for your response. My wife did have a myomectomy 2 years ago and had her uterus successfully reconstructed. Do you think or have you ever heard of this bleeding being related to her abortion causing an issue with her past surgery(this was a medical abortion, so no chance of puncture-i believe) We were well past the amount of time her DR had said to allow for healing.(over a year) My thinking is that if the BC is controlling her blood flow(although still bleeding) it couldn't be related to something damaged in her uterus. Thoughts??? We desperately want to start trying to have a child in 5 months...

Brief Answer: Unlikely to be related with past surgery. Detailed Answer: Hello XXXXX, Usually medical abortion is very safe in a scarred uterus like after Myomectomy, it wont cause this type of symptom. What I advise you is to wait for next periods, if it is still high go for further investigations like: Haemoglobin level (to rule out anaemia), Thyroid function tests (which can also present with similar complaints), Repeat scan of uterus-- to rule out any fibroids, endometrial thickness. Meanwhile I advise your wife to continue BC pills and the medicines which I advised after consulting your doctor. If you do not have any further clarification you can close the discussion.
