Brief Answer:
I have explained the working of the circulation.

Thanks for asking on HealthcareMagic.

I have gone very carefully through the details provided. I understand your concerns. The blood that comes back to the heart is ejected forward from it. The fraction of the venous return that is pumped forward in a single stroke is called the ejection fraction. So ideally it has to be somewhere around 100% in order to avoid development of backlog. So 24% is pretty low and the heart has a very difficult time dealing with the backlog that keeps piling. This leads to build up of blood (think like standing in a queue that is not cleared adequately and therefore keeps building up). In short this is what is called heart failure.

There are various blood pressure medicines that reduce the total amount of circulating blood volume and thereby reduce the load on the heart. I am sure that your doctor has given one of those. Other than salt and fluid restriction, there are no specific suggestions to be given. You have done a good thing in quiting smoking but should have done it earlier.

Brief Answer:
Regular exercise improves Ejection fraction

Detailed Answer:
Regular brisk walking will physiologically condition the heart and improve the cardiac output, thus improving the ejection fraction.
I do not think that you need a pacemaker unless there is a definite sort of arrythmia going on.
