Question: Hello Dr. Saha, this is Rosemarie Raia again, I spoke to you last month. I have been having the problem with the nausea and dizziness. The dizziness has subsided but the nausea persists. I am not taking the omeprazole because a lot of its side effects are dizziness, irregular heartbeat, and other things. I have developed a very dry mouth and went to the dentist. He referred me to a dental surgeon. I recently found out from a dental surgeon that my salivary glands are not making saliva. She wants me to get a ear nose and throat evaluation for the lack of saliva. Could that cause the nausea? My family doctor, who is in the office one day a week, keeps pushing the upper endoscopy the thought of which scares me to death. The GI doctor also keeps suggesting it. As you know I had the upper GI barium swallow which showed a few tertiary esophageal contractions and mild esophageal spasm, small hiatal hernia, trace to mild esophageal reflux and a mildly thickened gastric mucosal fold pattern. Could any of these cause the nausea when I eat? I have not tried the Metamucil and am not taking the omeprazole because I was afraid it was causing the dizziness. I have not had it since I stopped taking it. I am still in distress and I need your help. After I rated you, they closed our session so I am asking for your help again. Doctor, in your original reply you said that our gastrointestinal tract has a forward movement. If this
movement gets hampered, food and digestive juices would stagnate,stomach acid reflux into the food pipe and bowel habits would not be proper. How does the food get hampered? You also said you felt I had gastritis or gastroesophageal reflux which the Upper GI barrium swallow confirmed. Also, if i take the Metamucil, should I take it every day or just when constipated? I asked earlier about a problem I am having with dry mouth and the dental surgeon said my salivary glands were not producing saliva. Would that have anything to do with using fluticasone for nasal drip and using a cpap machine for sleep apnea?

Thanks for asking once again.

A hiatal hernia is the cause of reflux. The others seem to be less likely. I also think you should get the upper GI endoscopy done which they're also suggesting.

It would be a good idea to take Metamucil regularly and not just when you have constipation.

The less saliva could be linked to your sleep apnea and CPAP.

My neurologist wants to put me on Cymbalta. What do you think about that compared to the medicine you suggested fluxodine? Are they both antidepressants? I am now taking zanax .25 mg. half in am an half in pm.
If this test is to be scheduled soon I will need something. Can I take the xanax at the same time as the cymbalta or fluxodine? How long does it take to start working. Please answer as soon as you can.

Cymbalta would be a good choice

I would urge you to go ahead without any hesitation. Cymbalta is almost same as fluoxetine that I suggested. You may feel free to go ahead with any of of them. They are not antidepressants but do have antidepressant action. They would be better than Ativan in your case. However, they do take a few weeks to show action.

Dr. Saha, I am feeling very weak and nervous. I did not take the half of a .25 xanax today because on the side effects it says danger of suicide. Now I am afraid to take it. I also am afraid to take the omeprazole because it has side effects of dizziness, upset stomach, weight loss, not hungry, weakness muscle cramps etc, all of which I have. I have lost 13 pounds since July because of the nausea and not eating much because I am not hungry. I need some help soon. Please answer soon. Is there an antianxiety medicine that does not have the suicidal warning?

I had hot prune juice a few days ago and had a diaherra that night. Would not that have made all feces come out? If the movement was hampered,

Feel free to take Cymbalta and Omeprazole

I understand your anxiety. But let me assure you that the listed side effects occur in the only handful of people (something like 1 in 10,000) and so there is no reason to assume that you would get them. Just relax and take the medicines. I have been prescribing omeprazole for so many years and have not observed any significant side effects. Take Cymbalta. It is a good anti-anxiety medicine which you can take without any worry.

Skip the upper GI Endoscopy if you are apprehensive. Nothing needs to be done about a hiatal hernia. You can take all sorts of foods except spicy and fatty ones.

You need to keep taking fiber foods. Gradually the stagnation will clear. Till then you may have nausea.

Dr. Saha, if I take the Metamucil and eat the fiber foods you suggested, how long would it take before my nausea goes away. I am very weak from not eating much. I take the Zofran for the nausea but it does not take the fullness away from me and I still cannot eat much. The nausea is not as bad as it was but I have a feeling of fullness like as if I already ate a meal but did not. I do drink water. I ask for a answer soon because I am getting pressured from my family to get the endoscopy and I am tired of being sick. This has been going on since July 10.

If you take Metamucil it would still need a few weeks for your gastrointestinal system to regularize. Zofran is not expected to take your abdominal fullness away. It is due to bloating caused to to the stagnation I have discussed.

Dr., if this was Schatzki's ring, would that show up on a barium swallow upper g. i .
I told my g. i. doctor that I was afraid of the omeprazole. She told me to take a Zantac, 300 mg. in the morning and one at night. isn't that too much. She also told me to take Miralax because I told her I was concerned about the choking hazard on the Metamucil. The Miralax which I bought tonight says not to take for more than 7 days. If I had cancer would it have showed up on the upper g.i.? What does the Omeprazole do and what does the Zantac do? How do I know if I am making enough stomach acid? or too much? Thank you again for your answer.

Taking Zantac 300 twice daily would not be to much.
It blocks acid secretion using a pathway different from omeprazole.
Feel free to take Miralax. You need not mind the 7 day label. I am not concerned about choking hazard in case yu choose to use Metamucil.
