Answered by Dr. Sankaranantham Murugan (45 minutes later)
Brief Answer:
Sugget to get tested for HIV and rule out the possibility.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
All the exposures are not infectious. The girls you had encountered were not necessarily HIV positives.
Nobody, even agreat expert can not tell whether it is due to HIV or not by seeing such lesions alone without testing the blood for HIV. Body pain and red dots can occur in many situations other than HIV.Guessing things and odd possibilities would bring unnecssary mental tension. Rule out the possibility of HIV with appropriate tests.

Sir they r my girlfriend's, nd I don't know about hiv. Did u see dat 3 red dots on my hand? and last week I tested RVD (tri dot) it's showing negative. Please tel me sir
doctor
Answered by Dr. Sankaranantham Murugan (44 minutes later)
Brief Answer:
Your lesions are not related to HIV.

Detailed Answer:
Hi,
Welcome back.
They are your girl friends (known girls). Chances of HIV is low. The red dots are not specific for HIV.
Tridot done by 3 weeks after the exposure was not reliable. As you are in window period antibody tests are not reliable. You can go for antigen detecting test like P24 antigen test or DNA PCR, Qualitative RNA PCR are reliable and confirmatory to rule out HIV 1 infection by 1 month duration after the risk activity.

Is there any possibilitys to hiv? I don't know anything about hiv?
doctor
Answered by Dr. Sankaranantham Murugan (12 hours later)
Brief Answer:
Your symptoms are unlikely due to HIV.

Detailed Answer:
Hi,
Welcome to HCM.
Earliest manifestations of HIV can occur in the presence of an infection 4-6 weeks after the exposure. It will be usually a 'flu' like syndrome (fever, headache, body pain and throat infection). Rashes can occur rarely.
But not like dots in your hand. Your lesions are unlikely due to HIV. Whatever may be the symptom without a positive blood report HIV can not be diagnosed unless it is an advanced state where certain diseases specific only for HIV can occur.
When your girl friends are not HIV positive then you can not HIV from them.

Sir but I had a sex wit 2 girls in single week? that's y am afraid of this much. And my body full pain sir muscle pain, Not feeling hungry.
doctor
Answered by Dr. Sankaranantham Murugan (1 hour later)
Brief Answer:
Body pain and loss of appetite can occur due to anxiety also.

Detailed Answer:
Hi,
Welcome back.
Rather than sex with two known girls, sex with unkown partner or people with mutiple sex partners or sex workers are more risky.
Body pain and loss of hunger can occur with anxiety and in many other conditions also. Not necessarily be due to HIV.
But testing for HIV is the only answer for everything.

Sir I have enclosed lab report of p24 antigen test please interpret the result to me? Is this screening test is enough or should I do some more tests...because I had sex one month before, so will it detect or shall test some more?
doctor
Answered by Dr. Sankaranantham Murugan (5 hours later)
Brief Answer:
You are free from HIV 1 infection.

Detailed Answer:
Hi,
Welcome back.
You had undergone 4th generation DUO test (P24 Antigen+ HIV 1 and 2 antibodies). You had this test by 1 month after the exposure. This negative test by one month ruled out the possibility of HIV 1 infection. P24 antigen detection by 1 month is highly reliable confirmatory. Antibody part of this test can be reliable only after 12 weeks after the exposure (after the window period). HIV 1 infection is most commonest HIV infection(99.5%) throughout the world except in certain parts of XXXXXXX HIV 2 is rare (less than 0.5%) and can be detected by antibody detection test after the window period.

doctor
Answered by Dr. Sankaranantham Murugan (13 minutes later)
Brief Answer:
You are free from HIV 1 infection beyond any doubt.

Detailed Answer:
Hi,
Welcome back.
You are still in the window period of HIV infection. Antibody formation may be delayed up to 12 weeks after the exposure in the presence of HIV. So these tests cannot detect the presence of HIV infection during the window period. Presence of antigen particles of HIV can be detected earlier. P24 antigen part of this test can detect only HIV 1 infection. So your negative 4th generation test ruled out the possibility of HIV 1 infection. A rarest possibility of HIV 2 infection is yet to be ruled out after the window period.

Sir what's dat HIV 2? How it will affect? Plz tel me clearly about HIV 2. Any chances to affect HIV 2 to me?
doctor
Answered by Dr. Sankaranantham Murugan (1 hour later)
Brief Answer:
HIV 2 is less virulent and chances of transmission is low compared to HIV 1

Detailed Answer:
Hi,
Welcome back.
Prevalence HIV 2 is less than 1% throughout the world except in certain places of XXXXXXX It is less virulent, chances of transmission is less, so it is less prevalent. Otherwise sypmtoms and route of transmission are almost same as that of HIV 1 infection.
Your 4th generation DUO test one month after the exposure clearly ruled out HIV 1 infection already as P24 antigen was not detected.

Brief Answer:
HIV 2 infection has to be ruled out.

Hi, Welcome back.
A very remote possibility of HIV 2 infection has to be ruled out after window period or it can be done by now with HIV 2 specific RNA PCR test available only with selective laboratories.

doctor
Answered by Dr. Sankaranantham Murugan (5 hours later)
Brief Answer:
Nnondetectable P24 antigen ruled out HIV 1 infection beyond any doubt.

Detailed Answer:
Hi,
Welcome back.
What you had was not an exclusive P24 antigen test. It was a 4th generation DUO test. As I had already told that your P24 antigen part of this test is very much confirmatory, one month after the exposure. The negative test ruled out the possibility of HIV 1 infection.
P24 Antigen part of the test was able to detect only HIV 1 antigen and not HIV 2 infection.

But now on my chest I found red rashes .... This rashes not itchy sir, HIV rashes itchy or not? Multiple sex is cause HIV?
doctor
Answered by Dr. Sankaranantham Murugan (7 hours later)
Brief Answer:
No specific characters for HIV rashes.

Detailed Answer:
Hi,
Welcome once again.
There is no specific characteristic for HIV rash. All types of skin diseases which can produce rashes can also occur in HIV disease also and the severity would be according to the severity of the immune deficiency and it could be either itching or nonitching. Early acute infection rarely associate with generalised pink color rashes like that in measles or any viral fever with very mild itching or without much itching and not as papules.
Multiple sex is as such not infectious, if any one of the partner has HIV then you would have likely chances to get infection.

Hi,
Welcome back.
Different types of Herpes infections are there. Herpes Simplex 1and 2, Herpes zoster etc. Among which HSV 2 is sexually transmiscible whereas others can be acquired through casual contact. All these infections would unlikely be president scatteredly all over the body. I am not sure what your Dermatologist referred to. They are not necessarily related to HIV. All can be treated with appropriate drugs. Follow the advice of your Dermatologist. I am out of country for a while.

But without a positive blood report nobody can diagnose a HIV disease.
