Can HIV Be Contracted Through Non-penetrative Sex?

default
Posted on Tue, 12 Jul 2016
Question: HI Doctor
I had a possible HIV exposure on 7th may. Let me first explain the possible exposure. It was a MSM exposure. The guy sucked my nipples and then he lay on top of me and put his penis in between my thighs and did forward and backward thrusting. He did not penetrate my anus. I had a very small anal fissure right at the opening of my anus , and it could be possible that his penis had precum on it. But there was no penetration.
My questions are as follows.
1.) IS this a real exposure, in the first place, considering his penis could have touched my anus but not penetrated it, and with the fissure being there or BECAUSE there was no penetration , this isn't an exposure.?
2.) i did my HIV 4th generation test XXXXXXX ( p24 antigen & antibodies) on 20th day and it's negative. i WENT TO an hIV specialist here in XXXXXXX who said that it is as good as conclusive, as the p24 antigen would have been produced in 20 days, if it were to be, hence he told me to relax.
3.) However I also a fever of 102 , body aches, extreme stuff nose ( basic flu like symptoms that come with ARS on the 20th day itself) I had dolo and the fever went away and with 2 days of complete resting, i was fine again....
My question is
1.) Would the exposure i've explained, be counted as one, or was it a safe exposure, since there was no penetration involved?
2.) a 20 day p24 4th gen test is conclusive , as stated by the hiv specialist? or should i get tested again.?
3.) i continue to feel weak, light headed and a little feeling of vomitting is coming from today morning. aSLO I've been sweating a lot, agreed it it summe,r but even if im out for 5 minutes i start sweating profusely...

PLS HELP, I'M VERY WORRIED.

Brief Answer:
Don't worry as there is no risk of HIV.

Detailed Answer:
Dear Sumrit,

Welcome to HCM once again.

The exposure you had mentioned is safe as there was no penetration. You can consider P24 result as conclusive because there is no risk in your case but your satisfaction get tested again after 4 week.

The symptoms you have are due to excessive heat or summer nothing else so don't get worried about that. Take proper rest and diet, drink plenty of oral fluids to maintain the hydration and avoid going to outside in heat or direct sunlight.

As your friend I will also advice to stay away from unsafe sexual practices to prevent future risk of HIV or other STDs. Be relaxed and move on in life happily as you are absolutely safe and risk free.

Thank You

Hi doctor
Thank you for your reassurance.
I will get a test on 28 days again.
However, from scientific point of view and for my satisfaction, a 20 day test for p24 in general is a good consideration for a diagnosis right?
I mean in general, p24d would have been detected by the 20th day right?
Because the doc told me 28 days is more of an official guideline, however the tests are pretty sensitive so the window period is actually much shorter?

Brief Answer:
Yes definitely.

Detailed Answer:
Dear Sumrit,

Welcome back and thanks for follow-up.

You are absolutely right that P24 antigen would be detectable after 20 days so the test result is highly reliable and good for your satisfaction. Don't worry and get tested after one month as per guidelines.

Thank You

HI
Thanks for your continued support and guidance doctor, I'm trying really hard to get over this incident but today morning i woke up and saw these reddish rash/pimples on my chest.
Does this resemble AN ARS rash..
i don't want to go to a doctor again, since im already mentally very tired of this whole hiv scare.
I've attached a picture, please have a look.
Also what is ARS rash distinctly?
And is it very different?
doctor
Answered by Dr. Sunil Jangid (9 minutes later)
Brief Answer:
It is not an ARS rash.

Detailed Answer:
Dear Sumrit,

An ARS rash will appear after 15 days of exposure but not after such long period and the risk is also not there in your case so it is not related to ARS.

I have seen the attached picture and the rash looks like allergic or some other fungal infection. Consult dermatologist to isolate the cause and follow the treatment regime. Be relaxed and move on in life.

Thank You

Hi Doctor
How are you?
This is in follow up to my previous question itself.
I again took the 4th gen duo test at 26 days post exposure., and the result is non reactive. I told you my exposure wasn't technically one, considering there was no penetration, but the guy had kept his penis in between my thighs and I had an anal fissure at that time.
To say this fear of hiv has wrecked my life would be apt for now. I'm genuinely going through a lot mentally. I don't want to test again. The problem is I've constantly had fatigue, been feverish ( somedays getting 99ish fever) , and headache and had a constant constant stiff neck...

My questions stay the same, for some reassurance.
1.) Is there any possible exposure, in my given scenario? I mean could there be any cuts in the area b/w the scrotum and anus , like could the anal fissure cut be outside and it could have got affected?
2.) How conclusive is a 26 day 4th gen test? the official guidelines is 28 days, but is 26 days also as good as 28 days?
3.) Could I please talk to you telephonically once?

Brief Answer:
You are absolutely safe.

Detailed Answer:
Dear Sumrit,

Welcome to HCM once again.

There is no risk of transmission of HIV in your case and you are absolutely safe as the result of 26th day 4th generation test is negative. This is as reliable and good as 28 days result so don't get scared or feared about HIV.

You need not to repeat 28 days test now but get tested again after 3 months as per routine guidelines nothing else and the result will surely come in your favor. Forget about the incidence and move ahead in life happily.

You can ask me your queries on this forum any time and I am always available here to help you out. For any further details you can contact the customer care. Be relaxed and consider counselling services if needed.

Thank You

Sir
I'm trying my best to find assurance in your answer but i have just not been feeling well. For the past days , i've had a terrible headache just in the center of my head also, i've had a constant temperature i 99-100 range.
So the doctor i visited told me, in 26 days , p24 antigen is more often than not detectable so 4th gen in 26 days is pretty accurate but the symptoms just dont seem to die down also i don't want to test again. i dont know why is this fear there but i just dont want to.
In your experience has a 26 day negative result turned positive later?
Also why do you think im experiencing these problems.

Answered by Dr. Sunil Jangid (35 minutes later)
Brief Answer:
Just because of anxiety.

Detailed Answer:
Dear Sumrit,

Welcome back and thanks for follow-up.

The 26 days 4th generation result is highly reliable and accurate. I have also not seen any such case where it turned out to positive after 3 months so be relaxed and get tested again after 3 months.

The symptoms are due to excessive anxiety or fear or tension of HIV nothing else what I think. You can consider counselling services if needed. Don't get worried and move ahead in life happily.

Thank You
