Question: Hi Doctor,

My exposure date - 22nd Dec 2016
- Kissing , touching cheeks , neck of girl of unknown hiv or std's status.
- Nude bath in shower and kissing her while bathing and touched her vagina from hand.No Fingering inside.
- Sucked and licked her breast extensively
- Nude on bed , kissing and sucking her cheeks and breast on bed and finally penetrated on bed sheet( no vaginal touch and lip locking at all)
- She had some dry scar or white rash at ribs near neck and may be by mistake i have touched from my hand or lips.
- uthera contact to bed and may be her leg or stomach but not vagina.

My test Results

Hiv 1-2 elisa - neg ( 15 days)
Hiv 1-2 elisa - neg ( 51 days)
HIv 1-2 Elisa CMIA ( Dr lal path Labs) - Non reactive at 120 days - Value 0.22
HIv 1-2 Elisa CMIA ( Dr lal path Labs) - Non reactive at 220 days - Value 0.06
Hep B & Hep C - Neg (120 days)
Herpes 1-2 - Neg (120 Days)
Vdrl - Neg (120 days)
Claymadia - neg (120 days)

Now The important Point:

Last week I went to XXXXXXX and got wet during the rains. Next Day I got fever/flu (Full Body Pain , temp in between 102-103 F , cough & Shivering , red dots at lower leg from last Wednesday ( 10-08-2017 till 14-08-2017). After medications (Paracetamol) fever has gone but still cough is persistent.

Now major complications :

1) After fever went , from till now I am not experiencing appetite and lost weight of 2 kg in last 10 days.
2) Feeling very weak ,light headiness , blurr & weak vision.
3) Major important thing .... I am getting white discharge near penis area.
4.) If eating anything , than rushing to motions every time. Feeling tightness inside stomach.

"I would like to share that during my multiple test till 7.5 months , i had multiple sexual intercourse with my Wife in between " Does it considered as exposure or not as in multiple sites its mentioned no sexual exposure during the tests."

I) doctor is the last week fever is just a Flu or my body is sero-converting , I hope test upto 6 months are conclusive??

2) Even after 7.5 months negative hiv antibody test and kind of exposure , should i still worry about hiv or any other STD.
3) sexual exposure with wife will create any impact on test results or not ?
4) is the loss of appetite , blurr vision , weight loss after fever , fatigue is sign of hiv symptoms or anxiety?

I am Dr. kakkar (dermatologist and venereologist). I have read your query in detail and I have noted your concern.
First of all, the nature of physical contact that you had in DEC 2016 does not confer any risk of HIV.
HIV is only transmitted through either vaginal, anal or oral sex. Not through touching, kissing, rubbing etc. Also, the HIV virus does not pass through skin rash.

Sexual contact with your wife would not have interfered with the seroconversion.

It is mentioned that prior to HIV screening there should not be any recent RISKY sexual exposures I.e sex with partners of unknown status, so as to allow correct interpretation of the test result. For example, if someone acquires HIV infection just before a screening test, it will not be detected by the screening test and the person would be under false impression that he / she is HIV negative, whereas the actual reason is that the screening tests simply was unable to pick up very recent infections. There is usually a window period of 2-3 weeks after HIV infection during which a screening test is unable to detect infection. This is the reason for asking one to refrain from having RISKY sexual behavior at least 4 weeks prior to HIV screening because otherwise the test result would not be conclusive about HIV status. However, in your case it was NOT a RISKY sexual behavior.

You had sex with your wife who is your trustworthy sexual partner therefore your test results are reliable/valid. Having sex with your wife did not affect the interpretation of test results.
You should not worry about HIV as it has been conclusively ruled out. Screening test results are reliable post 1 month of risky sexual encounter. So at 6 months or at 7.5 months they are definitely reliable!!

Your recent symptoms are perhaps related to flu and some of them are due to stress and anxiety.
You need to tell me more about the white discharge near penis. Are there any symptoms like itching?burning? Is the discharge coming from urethra?

Hi Doctor,

The white discharge is not coming from Uthera but very small portion got accumulated near the foreskin and that too started from 1-2 weeks back.

There is no itching and burning at all.Just a little discharge only on foreskins and sometime on tip of penis.

I had very long episode of hiv anxiety from last 1 year and middle of it this expsoure on Dec happened.
Althought i was knowing the fact that there is no penetrative sex happned with CSW and i was relaxed. But after that such symptoms started and made me more anxious.

Now from my mind i am not able to understand and diffrentiate that such issues of not feeling hungry(lack of appetite) , cough , feeling chills , fatigue and light headiness are due to flu and anxiety or its still sometime related to hiv infection( as all such symptoms are written so much in multiple websites).

Me and my wife has abestine and trustful relationship to eachother , so in that case if all test till 7.5 months are conclusive than why such sypmtoms again happening in my life. Pls suggest

hi.

Perhaps you have a slight yeast infection of underside of prepuce and glans. I suggest you to use a topical antifungal, antibacterial cream combination e.g candiderma plus cream (contains beclomethasone, clotrimazole and neomycin).
Your symptoms are certainly not due to HIV. However I suggest that you also consult an internal medicine specialist for these symptoms and also get a complete body check up tests from a lab.

Thanks doctor for vote of confidence.As last follow up i would like to ask. DO i need ever to retest for any STD like hiv , hep B and C , herps etc in coming future considering the last episode on Dec 2016.
I have consulted my case with two internal medicine doctor and they are saying its not related to STD and asking me to consult to psycological treatment.

But if its psycological , then how i have lost my appetite just after fever of 3-4days.
How i am experincing problem of upset stomach , stomach and chest heaviness , hot and cold flashes.

Is all such physical problems are due to adverse effect of prolonged anxiety and stress ( causing my immunity to getting weaker) or something related to std disease.

My wife is asking me everyday for unprotected intimisy to plan another baby and my mind is not free and scared of STD due to such symptoms.

No. You don't need to retest any further. Your results are conclusive. Moreover none of these symptoms are specific to any of the STDs.
Anxiety and stress may have a role to play in contributing some of these symptoms but you need to now believe that you don't have STDs.Coordinate with your internal medicine specialist and hopefully he/ she would be able to find out an alternative explanation for these symptoms.

Hi Doctor,

Yesterday I visited my Internal medicine doctor again and he also suggested of no worries but while taking he mentioned that infection incubation period is upto 1 year to spread in once body.

Just in curiosity would like to ask , if there is any need to 1 year final testing required or not?

Secondly I am getting serious soar throat from yesterday onwards does it anything related to my fear or may be due to atmospheric change.
