Hello and welcome to HCM,
As evident from your history, your father is suffering from tuberculosis and anti tubercular treatment (ATT) has been started.
ATT takes some days to control the symptoms.
Fever and sputum production may continue for some days but are usually controlled by 15 days and the person becomes non-infectious in one month time.
Sometimes after starting the ATT, fever may flare up.
This effect is due to accumulation of breakdown products of mycobacterium tuberculosis (bacteria causing TB).
Sometimes opportunistic infections may also flare up.
However, there is nothing to worry if any of these problems arise.
In case of opportunistic infections, appropriate antibiotics can be used after consulting your treating doctor.
You can wait for 15 atleast 15 days for the fever to fade out.

Thanks and take care

General & Family Physician Dr. Manjeth Kumar G's  Response
Hello,
Welcome to Healthcaremagic.

I suspect it may be a relapse of typhoid fever or it could be a other infective cause not only the tuberculosis.

I need some clarification.
Whether typhoid is confirmed by widal as well as blood culture?
Whether he took antibiotics properly and completed the course?

If not then it may be a relapse of typhoid or other infection.
Usually some patients discontinue the antibiotics when they feel better even before the completion of full course.

DO the above said investigation. In addition, do Complete blood count, Peripheral smear for malaria, urinalysis, blood culture.

Please do an ultrasound abdomen, to rule out any organomegaly.
If you suspect tuberculosis, Do sputum examination and mantoux test.

Consult a Physician with these reports.

Hope this helps.
Take care.
