Child Taken Calpol And Antibiotic For Cough And Fever. Montoux Test Positive. Chest X-ray Normal. Advice?

Dear Doctors,

My Daughter is 5 and Half Year Old as on. For Past One month she was suffering from Cough ( Khansi) and she was treated for that by Child Specialist and give her some Antibiotic and on 25th August she was suffered by Viral Fever and we have give her Calpol 250 for 4 days and the same day her doctor advice to do Blood Test e.g.CBC, Vitamin D, Total IgE and X ray of her Chest.

Her Blood Test Results shows the following:
1. Haemoglobine: 11.9 2. RBC Count: 4.96 mill/cu.mm 3. WBC Count: 8700 4. Platelet Count: 307 x 1000 /
5. MPV: 8.5 fl 6. Hematocnt: 37.2% 7. RDW: 13.2% 8. Neutrophils: 63%
9. Lymphocytes: 34% 10. Eosinophils: 3%
11. E.S.R: 10 mm/hr 12. SGPT: 18 U/L at 37 Degree C
13. 25,Hydroxy Vitamin D: 11.80
14. Total IgE : 310.89
15. Chest X Ray: Normal X Ray Chest. and
16. MONTOUX TEST: Tested on 25th August = And AFTER 72 Hrs it shows 17 mm .
17. Weight: She reduced 1.5 Kgs in past One Month 19.5 Kgs to 18.00 KGs AS ON

As per the MONTOUX TEST she is POSITIVE for TB, but Our Doctor is little doubtful as she is saying that her
MoNTOUX Test is Positive but When We co-relate with Reports she doesnt have TB, but came to conclusion that " We have to do the MONTOUX test on her other hand to

Can you please advise on the above.

Thank you for your query on Healthcare Magic.
Mantoux test of 17 mm is positive and it is suggestive of latent tuberculosis.
In latent tuberculosis, the x ray usually normal and there will be no symptoms.
But loss of 1.5 kg in the child weight is significant.
Latent tuberculosis requires treatment with two drugs of anti tuberculosis medication for 6 months.
I advise you to discuss your concerns with your pediatrician and if necessary take a second opinion from a senior pediatrician or an infectious specialist.

Hi,
The Montoux test +ve with17 mm induration indicates NO risk factor for
T.B.( CDC classification). So your child does not seem to have TB infection.
I don't think testing on other hand will have any significance.
Low grade fever,loss of appetite, weight reduction , low grade fever with evening rise , lymph node swellings in neck are other signs -which must have been excluded by your doctor.
The weight loss is usual with viral infections/ long standing illness.
If at all you want to confirm,IGRAs ( Interferon gamma release assays)test can be done.
She needs rest, high protein nutritious diet, vitamin & iron supliment to come out of the weakness.
