What Does This Thyroid Profile Test Report Indicate?

Question: Hi i have hashimoto's thyroiditis i went for my lab check up and it came back abnormal and i cant see an Endo for a while so id like some answers now since it will be a few weeks before i can be seen.

All labs are normal aside from T3 Total
TSH: 0.49
T4, Free: 1.4
T3, Free: 2.4 normal (but seems lower end of normal id say)
T3 Total: 261 (way above normal

all my other labs are ok as well my CBC is fine. WBC RBC etc all good ranges. No anemia good ferrtin levels etc are good.

My thyroid area DOES ache a little. but nothing too crazy. im slughish. brittle nails dry skin brittle hair etc. but that is normalish for me. but it has gotten worse in the past few months.

i have googled and can find NOTHING on everything normal besides elevated T3 total.

Sorry to note your symptoms. They suggestive of 'hypothyroidism' ie an under-active or inadequately treated thyroid.

I see you are on synthroid, and have been for numerous years.

As you have surmized, your thyroid labs are all normal except the Total T3. There can be a few reasons for this. I will describe one here.

When the TBG (Thyroxine Binding Globulin) in the blood (which normally carries T3 and T4 along with it) is present in higher than usual amounts, it can lead to a higher blood level of Total T3 and typically T4 also.

A common situation in which this occurs is pregnancy, especially the latter part of it.

Other than this, there are some genetic abnormalities where such carrier proteins like TBG are present in excessive amounts and thus result in higher levels of Total T3 and Total T4.

In such circumstances, Free T4 levels give a better idea of the thyroid hormone level status in the blood.

This being said, thyroid matters are fairly complex and it is best left to the expert ie an endocrinologist to make final assessments as to the exact cause in each individual. Such a comprehensive evaluation requires a thorough physical examination in-person too.
