Question: Hello Doctors
I am undergoing treatment of Hepatitis "A". My bilirubin was 8.3 and SGPT -1490 and SGOT -410 after 15 days again it measured Bilirubin - 1.42, SGPT-120 and SGOT-78. Now yesterday again it measured Bilirubin 0.78, SGPT-132 and SGOT - 70 and other all measures in reference rage like Alkain Phosphate, GGT, Albumin etc. Please let me know thatbilirubin is normal level but why SGPT and SGOT not control? Already done USG there is no other problem and also lever size is ok.

Brief Answer:
SGPT,SGOT are highly sensitive liver function tests, may take some time..

Detailed Answer:
Hello,
Welcome to HCM,

I have read your query.

Your billirubin 0.78 is completely normal while SGPT-132 and SGOT-70 are marginally elevated (but not too high).

Both these LFTs (SGPT,SGOT) are highly sensitive liver function tests,so they might get easily raised following oily or fatty meal or even few alcohol drinks or sometimes following use of NSAIDs pain killers or heavy exercises.

Since you are undergoing Hepatitis A treatment,we may expect to see these LFTs high readings (slightly high) even after completion of treatment and it may take another 3 or 4 weeks to come down to normal.

So, do not worry about it and possibly avoid fatty/junk/oily foods further or alcohol if you take.

Also, your other liver function tests like alkaline phosphatase,GGT and albumin are within normal limits with normal ultrasound abdomen, I do not think there is anything to be worried here.

You can repeat the test for SGPT and SGOT after 4 weeks on fasting blood sample and most probably they will come within normal limits.

You may also take tab.Udilive 300 mg twice daily meanwhile after consulting to your doctor.

Syrup LIV-52 DS, a herbal preparation having hepato-protective property, may also be beneficial for you.

Hope this helps and let me know if you have more questions.

Brief Answer:
Please discuss with your treating doctor.

Detailed Answer:
Hi,
Thanks for the follow-up,

As you are undergoing treatment from 15 days and there is no clinical symptoms at present with normal liver function tests, there is absolutely NO contraindication to resume your work.

But I would suggest you to discuss the same with your treating doctor as he/she knows your condition well enough.

You can start your regular activity slowly but to resume your job it is best to ask your treating doctor before acting upon.

Regards,

Dr.Sandip Kabra

Hello Doctor,

I am very satisfied with your reply and please note that my doctor also prescribed same medicine Udilive 300mg and again after 15 days i tested LFT and all the report is ok as per last but only SGPT and SGOT elevated 120 and 62. Now doctor already again tested USG and sugar etc but only cholesterol level is 300 and tryglyceride is 430. Now doctor give fitness certificate and prescribed Udiliv 300 for 15 days and lipicard 160 for 15 days. Now I have joined my duty and 900 km away from my native place and it is not possible to go again to native place. Doctor assured that it will get cure not require to appoint again but after 1 month i need to again LFT and myself I can check and if it is ok then no problem. My question is may I stop Udilive 300 medicine after 15 days or continue it? You also suggested Liv -52 DS Syrup. I also heard that Liv 52 Ds strong liver. So after 15 days can I take Liv-52 DS? if yes then what doase it will be?

Brief Answer:
Continue Udiliv 300mg even after 15 days. Take Liv 52 DS 2TSF daily

Detailed Answer:
Hello,
Thanks for the follow-up and sorry for the delayed reply,

I have read your follow-up query,

Yes,you can continue Udiliv 300mg even after 15 days till you check liver function tests again i.e. after 1 month from now. It is a bile acid, produced naturally by the body and will also help to reduce your blood cholesterol level.

You can also take Liv 52 DS (even along with Udiliv) 2TSF daily which is a herbal preparation and beneficial for the liver. Yes,I would be in favor of taking Liv 52 ds syrup following hepatitis treatment.

You can check the slight reduction in SGPT,SGOT (120,62) values as compared to last reports. So,I would suggest you NOT to worry about liver function tests. They might take some time to get back to normal as we discussed in the first conversation.

For high cholesterol and triglycerides, yes you should continue to take tab.Lipicard as directed by your physician.

Check SGPT,SGOT and lipid profile exactly after 1 month from now.

Most probably, liver function tests would come down to normal and in case,if there is still high value of blood cholesterol and triglycerides, then it is better to discuss to your treating doctor on phone,if appointment is not feasible.

Since you have resumed your job and feeling fine, there is no need worry about liver function tests.

Hope this helps. Let me know if you need anymore clarifications.

If LFT will be normal after 1 month then also i continue all the medicine along with this syrup or stop immediately?

However again I open this discussion after a month when I have report in my hand if it in range or not i will contact to you definitely for future prevention.
