Question: I am 41 years of age. I had always had a regular cycle of 28 days. My partner and I started trying to concieve in July 2013. This month was my first time having a short cycle of 19 days. I had just moved out to Thailand and I thought it was due to time change. I have had irregular- short cycles ever since. My GP was taking day 21 progesterone blood tests which were low and got lower. Last month I missed a period and had a cycle length of 60 days. I requested a transvaginal ultrasound. conclusion: Low echogenic lesion in liver with small peripheral echogenicities could represent a small endrometrioma. The lesion in the left adnexa measuring up to 2.1cm could represent a pedunculated fibroid or broad ligament fibroid. Repeat ultrasound is arranged for 2nd January. The following week I had a hysterosalpingogram. I found it painful and fainted during procedure. No abnormality in uterine cavity. Both fallopean tubes filled to the level of the outer third on both sides. Both fallopean tubes are extremely narrow and no further filling of the tubes occured. Maximal distention of the uterus was obtained until patient was unable to tolerate further pressure. Appearances suggest blockage of the outer third of both fallopean tubes.. Please could you give me your medical opinion on this. Is it possible that the tubes are patent and it was spasm because I was fainting? Also what due you think most likely cause of not getting pregnant? Also what treatment options would you recommend?

I read your query and I understand your concerns.

Following is my reply:

1. Most likely cause of not getting pregnant is probably your age.

2. It is difficult to conceive after 35 years of age and still more difficult after 40 due to reduced egg reserve in lady.

3. Blocked tubes could also be a reason for not conceiving but spasm cannot be ruled out. You may need to repeat HSG to confirm it..

4. Hence I recommend you to go ahead for IVF procedure immediately so that you save time and age.

5. Thailand has good IVF centres and might be cheaper also.

I have a half sister that got irregular cycles at same age as me. She had a cyst and once it was removed she was ok again.. Is it possible that the cyst was affecting my hormones and ovulation.. Also my partners sperm mobility was poor at 2 ..! We have now split up so I'm in a tricky situation.
Also I am not in Thailand any more I was only there for three months .. I live in north west of XXXXXXX . I don't know where is best for me to go for fertility treatment?

Partner's motlity, count and your egg reserve are likely problems not cyst

Hi,

Cyst may not have made much difference. If your partner's motility and count were low, it would have been a problem along with your possible decreased egg reserve..

You can search for good IVF centres near your place. Ask few people or google up. If you find acentre, please go and check out if you are eligible for IVF as you are 41. They will test your egg reserve and let you know about the possibility of entering into IVF programme.
