Question: Hi doctor , please find my semen analysis report
Volume 3 ml,Liqufication 30 min , viscosity normal , pH 8.0, reaction alkaline, fructose positive, sperm count 32.5 million / ml , rapid progressive motality 20 % , sluggish progressive motality 20 %, non progressive motality 10 % , no motality 50%, morphology 70 % , pin gaint and amorphous head 30 %, WBC 2-4 / hpf , RBC 1-2 /hpf, epithelial cells nil, sperm agglutinates nil

Welcome to HCM.

After having gone through the uploaded result your semen analysis it seems to me that there is a problem with the reduced overall motility of the sperms in particular the rapid progressive motility which are less than the normal range.

Ideally, the percentage of properly moving sperms in a semen sample is 50% or more. Sperm motility is the capacity for sperm to propel themselves in a forward direction for a sustained period of time to reach the egg for fertilization.

Since the rapid progressive motility of the sperms is reduced hence the chances of pregnancy are also found to be very low.

In order to increase the motility of the sperms, the following measures are useful:

1. Avoid alcohol, smoking or stress
2. Take healthy balanced diet rich in protein & vegetable, low in fat, fruits, minerals
3. Avoid hot baths, hot tubs, or using lap top on your lap for long time
4. Wear loose comfortable cotton undergarments.
5. Keep your weight in check and do regular exercise

Protein rich diet like meat, fish, poultry (if you are a non-vegetarian), otherwise pulses like lentils, whole grains along with fruits and vegetables are useful.

Dry fruits like walnuts, pistachio, melon seeds are also effective.

The motility of the sperms as per your report is as follows:

1. Rapid progressive motility 20 % &
2. Sluggish progressive motility 20 %

The extent of progressive sperm motility is related to pregnancy rates.
As per the WHO (World Health Orgnanization) guidelines the normal motility of the sperms required to cause pregnancy are as follows:

1. The rapid progressive motility should be more than 25%
2. The combined rapid & sluggish progressive motility should be more than 50%

The results of item 1. in your case is 20% and item 2. is 40%
Both these results are lower than the normal as per the WHO guidelines.

Now, may I ask you the following questions?

1. Are you married? If yes, how many children do you have?
2. What was the purpose of doing semen analysis?
3. Is it requested by your doctor?
4. Do you have any other problem or medical illness?
5. Do you smoke or take alcohol?
