Question: cat scan CT chest with contrast back and posterior rib pain status post fall
lung fields demonstrate mile/moderate linear subsegmental atelectasis versus scarring within the lingual. No evidence of pneumonia or pleural effusion. There are small scattered mediastinal lymph nodes, none of wich appear patholically enlarged.Central pulmonary arteries are grossly patent central large airways are patent liver is mily enlarged measuring 22cm in sagittal length this may be on the basis of hepatic steatosis given low attenuation of liver compared with spleen. The spleen adrenal glands, pancreas , gallbladder and bile ducts are unremarkable there is a 2cm cortical cyst within the lower pole of the left kidney and additional subcentimeter cortical hypodensity within the mild left kidney which is to small to charachterize. The kidneys enhance and excrete symmetrically. incednetal retroaortic left renal vien
there is large to moderate amounts of retained stool throughout a nondiliated redundant colon. normal appendix is noted There is trace amount of simple appearing free fluid within the cul de sac which is non specific but likely physiologic There is mild subcutaneous fat induration at midline along the low back and
related to poso a lesser extent along the right hip, presumably.
there is more in the full body ct scan but I do not understand much of it and what I may have goingwrong with me in multi areas

1. The lungs show changes which are likely to be small airway collapse in the lingular segment (left lobe).

2. There are small lymphnodes around the heart. These are scattered and are to be considered as a normal finding.

3. The liver is enlarged and measures 22 cm. This is a concern and needs to be discussed with your doctor. Liver function tests might be required as there is also fatty change in the liver.

4. The spleen, adrenal glands, pancreas , gallbladder and bile ducts are normal.

5. There is a 2cm cortical cyst within the lower pole of the left kidney and additional subcentimeter cortical hypodensity within the mild left kidney. A cyst is a water containing thin walled structure that is benign. As the cyst is below 3 cm therefore there is no need to worry. Kidney function is normal.

6. There is large to moderate amounts of retained stool throughout a nondiliated redundant colon. Normal appendix is noted. It means your bowels were not completely evacuated on the day of CT scan. However the appendix is visualized and is normal.

7. There is a small amount of fluid collection behind the uterus and can be accepted as normal.

8. There are certain patterns of the fat under the skin in lower back along the mid line. This is not a serious concern.

The following is suggested.

I. Having a mildly fatty liver on ultrasound scan is not a serious concern and can be due to many causes including the following.
(i) Increased intake of fatty foods and alcohol
(ii) Over weight and obese patients.
(iii) Those who have fatty liver running in family members.
(iv) Any liver condition that is changing appearance of liver.
(v) Mild fatty liver can also be there in many normal people

II. You might like to get complete liver function test done including SGOP, SGPT, Alkaline phosphatase, GGT, bilirubin (direct, total and indirect) done to make sure your liver functions are normal.

III. Doing lipid profile test to know if you have raised cholesterol level and any associated risk of heart problem.

IV. Discuss your reports with your doctor. Please make changes to your diet as per your liver and cholesterol reports and take medications if given.

It is good that you do not smoke or drink alcohol. From your height and weight the calculated body mass index is 29.05 and this means you are at upper end of being over weight.

Having a healthy diet and fitness schedule might help in weight management.

Fatty liver can be due to non alcoholic fatty liver disease. Under normal circumstances, if liver function tests are normal then nothing needs to be done. However it is suggested that you get an ultrasound scan of your liver every year or at least once in two year with the liver function tests to know if there is a concern and start taking measures to protect your liver function.

Question what is treatment of this an causes? What are symptoms and to know if they are worsening?
"likely" what brings conclusion of likely?
I have another qeustion
Does any one who has been hit with a blunt object that casued unconsiosness and stitches a traumatic brain injury? This is the definition given. If not what is diagnosis of a T.B.I i was hit with a bat over approx 10 years ago causing the described symptoms and have been diagnosed with a T.B.I

I have told my doctor i have pain in what i thought was my kidney at our last visit it was severe stabbing pain on the right side then moved to the left.
after a day it was oddd or idk what do i know i cant seeem to get answers :(
i was told that my kidney was not on my right side under the ribs is where it hurt and he ordered an ulta sound of my gaulbladder.
i do not understand why as the blood work has shown pre diabeitis and then diabities on the cusp .
i was told to take no meds by this dr and by another i was told to take metforman and use the prick test to keep blood sugar levels(track)
new dr said i do not need.
i ened up in hospital as i fell and had been in the hospital for4 days approx about 4 months ago they did an catscan and i was told i had tree buldging disks and something i cant remember that was thinning of the spine as well as degenerative disk disease ( that i was told is normal in peopel at y age (48 ) and that we all have it at that age.
After the fall over theweakend i was given a fulll body scan and i wanted and mri and was denied ( my insurance covers it no reffereal).
i am concerned about the increasing levels of glucose and the hypothyroidism i have had and recently ( last 4 months has worsend though dr has increased levotyroxine doses every month after new blood work started at 50 then to 75 then to 100 and now at 150 i believe and the results get no better/
i could not walk when they had me leave the hospital it was a terrible experience i am used to the hospitals in massachusetts mass general and brigham and womans in particular and the doctrs.
i am rural now and they do not seem to care much about hippa but thats another story ( every one is related to everyone from the pharmacist knowing the doctors and giving info i was uncomfortable with about who was married to whom. as i wondered what they would say of my info if they where fine sharing that which was none of my business.
i just want to walk and stop feeling pain the dr has me on oxycotin ten milligrams one ever 12 hours and percocet 5milllograms one every 6 hours i been on for months and i asked to reduce it even though the pain was not better , before my fall as it has its own problems . but i cant take the pain i am ready to go to the hospital now if i knew one that would give proper care.
advice? prognosis and other ideas on diagnosis thoughts?
also what would a a mri show that the scan i had didnt? and what if any would be the benifits.
please tell me your speciality and how what your education is please in your response. thank you

Please find my observations below.
1. The Ct scan finding is due to the small air spaces collapsing under your body weight when you were getting the CT scan done in a prone lying position. Since the lungs contain air spaces, they will slightly collapse in an overweight individual. When you stand upright, it gets back to normal. This requires no treatment and is not a disease. By likely I meant that the probability of air spaces getting collapsed is greater than scars, which was mentioned as the other cause.

2. Traumatic brain injury is defined as any injury to the brain which has caused symptoms ranging from slight confusion to severe brain injury causing death. The cause for such injury being direct or indirect trauma. A hit with a blunt object causing unconsciousness and requiring stitches is certainly traumatic brain injury.

3. Doing an ultrasound scan is one of the best investigations. The kidneys are also seen when we do ultrasound scan of the gall bladder. Your doctor wanted to do a complete evaluation for your pain.

4. Early diabetes can be controlled by taking proper foods. Your new doctor wants to know if your diabetes or high sugsar level can be controlled by diet alone and that is the reason they told you to stop medicine.

5. The fall has probably caused the discs to bulge more. At your age many people in the overweight gropu may have various degree of disc bulging problems. Mild disc bulges are not a serious concern and can be treated with pain reliever medicine.

6. Thyroid problems need continuous monitoring and dose alteration in medicine. Please schedule visits to Massachusetts when possible and meet the doctors you are familiar with.

7. Sharing info about marriage is not a good idea. However in rural situations in the community are different and they tend to share personal information about other members in the community with no obvious intention to harm anyone. This has been there since years and you can ignore such incidents of information sharing.

8. You can talk to your doctors and inform them of your fall and subsequent increase in pain. Medicines are the first line of treatment and if they don’t work well then you might require injections and nerve block on your back where the discs are bulging. This might not be possible in rural and you can discuss in detail when you see your doctors in Masachusetts. Prognosis depends on the amount of disc bulge, though there is no threat to life, pain is a big concern. You can try and reduce your weight and this will improve your pain.

9. Disc problems are best seen on MRI and it is recommended that you get one done if you are having significant pain that is disabling and not allowing you to do your daily activities. Please make an insurance referral and then proceed.
