What Do These Fasting Blood Sugar Levels And BP Readings Indicate?

Question: I am a diabetic patient , 72 years age, XXXXXXX Male, controlling with drugs. HbA1C was 7.3 two months ago. Current fasting blood sugar is 106 mg/cc & 2hrs PP is 148 mg/cc. Currently taking Janumet 50/1000 mg + Amaryl 1 mg before breakfast, voboose 0.3mg during lunch & again Janumet 50/500 mg before dinner.
I have also hypertension. BP is 135/85 mm of Hg. Pulse 90 /min. Having Telday H 40 in the morning after breakfast & Cilacar 10 mg + Ecosprin AT 150 after dinner.
Also take Veltum Plus due Prostatomegali but low PSA. PLEASE COMMENT IF OK

The target values at this age is :

Fasting sugars less than 130 mg/dl
After food less than 180 mg/dl
HBa1c 7-7.5%
BP less than 140/90 mm hg

You are taking good medications and the parameters are in target. So no need to worry.

Hope your cholesterols are also controlled. Target LDL cholesterol is less than 100 mg/dl.

Dear Dr. Ajish,
Cholesterol was last checked on 10th Oct 2014.
Total Chol 140 mg%, HDL Direct - 34 mg%, LDL direct - 80 mg%
Triglycerides- 147 mg%
Taking Thyroid medicine Thyronorm 12.5 microgram in the morning
Present TSH is 3.55 units
Hope all these are in Target range.
Also have Arthritic pains in Lower Hip joint & knee joint. Stopped Praedmet 4mg & Sazo 500 as advised.
HAD NOCTURNAL PANIC ATTACK LAST WEEK. HAD DIFFICULTY IN BREATHING FOR A MINUTE OR TWO TILL I SAT UP. NOW OK. Any extra checks needed?

Your lipid profile is normal. TSH Is also Ok.
So at present no need to worry. Please have an ECG done since you had breathlessness at night.

Tests needed on follow up are:

FBS, PPBS atleast monthly
HbA1c, TSH every 3 months
Lipid profile, renal function tests every 6 months
urine microalbumin, diabetic eye test, neuropathy tests every year

Dear Dr. Ajish,
I had my ECG done about 3 months ago when there was a sudden onset of vertigo in the evening followed by vomiting and not able stand for some time.
I also had a black out 7 months when I was in Swiss Alps. It lasted about 10 minutes. Angio tests & comprehensive health check up results on return showed complete normalcy.
any other check up needed like HCRC etc?

Welcome back.

Sudden onset vertigo and giddiness need to be evaluated. Usual tests that we do are:

1. Cardiac - Holter monitoring, positional tests etc can be done after consulting cardiologist.

2. ENT- to look for positional vertigo, clinical tests are there

3. EEG in selected cases - if there is loss of consciousness.

Unfortunately these tests can detect the cause in less than 50% of cases as in your case. Most cases will be treated without identifying cause.
