What Do The Following Blood Test Results Indicate?

Question: I recently had blood work done and am not sure about a few of the results. Can someone go over the results and let me know why the BUN is high? Also, I am having some sort of issue with either a severe drop in my sugar or a severe spike in my sugar. I am not sure which one it is. Although my sugar and glucose levels seem to be in a good range, I'm having issues when I eat sugary foods.

noted your lab reports. your blood profile including sugars are good. BUN goes high when there is dehydration. Not to worry about it. your creatinine and eGFR are great.

Thank you. Yes, I can elaborate on the symptoms.

First of all, I eat very healthy. I am lactose intolerant so my intake of sweets is very limited. However, I do indulge in sweets from time to time. The symptoms usually come on later in the evening.

If I eat too many sweets at one time, about an hour later I will become very dizzy, nauseated, profuse sweating, and sometimes disoriented. The last episode I had I was very disoriented to a point where my husband wanted to take me to the ER. The symptoms are not quick to subside. I usually feel like a complete train wreck the following day. Mind you, these symptoms are fairly new.

I wondering if I need a six hour glucose tolerance test?

yes, you may need an extended glucose tolerance test. the symptoms are quite suggestive of reactive hypoglycemia. this may be due to excess insulin secretion from pancreas. if you do your post fed insulin levels, they are likely to be high.
yes I agree that you need an extended glucose test following a mixed meal.

Thank you. I thought so as well as I Googled the symptoms and that is what came up. Could this be a result from the acute pancreatitis (Idiopathic) I had about two years ago? I've never had these symptoms until just recently.

Exact cause unknown

Detailed Answer:
The exact cause is not known. However reactive hypo can occur few years prior to developing diabetes. This is my experience. .There can be problems associated with first phase insulin secretion. then there is an exaggerated seond phase insulin secretion. Most of these patients develop diabetes later in life and ususlly have a family h/o diabetes as well.

Thank you again. One more question, if I may. I recently had an Abdominal Ultrasound which revealed "atherosclerotic changes of the abdominal aorta. There is no evidence of aneurysm." I recall my mother having this issue with her heart after many years of being a brittle diabetic. My mother's diabetes was caused from having cancer. There is no other family history of diabetes in my family. She was the only one.

Sorry, I wanted to ask could those changes be related to the reactive hypoglycemia or pre-diabetes?

Sorry for the delay in reply. Atherosclerosis is thickening of inside of blood vessels and cholesterol deposits with in it. it's a common finding beyond middle age . However it is true that diabetes accelerates atherosclerosis.In people with diabetes, atherosclerosis can occur at an younger age. you should get your lipid profile done and if abnormal, please consider starting cholesterol lowering drugs.
