What Causes White Spots On Swollen Tonsils Of A 3 Year Old?

Question: i have a 3 year old who has swollen tonils with white spots, step throat was ruled out by the doctor on Wednesday, he has had a fever ever since, throat still looks the same, he has a cough, and his nose is constanty running thick yellow discharge.

Hi...looks like viral pharyngotonsillitis. It will usually settle in 4-5 days. As strep is ruled out by rapid test, nothing to worry. But oral intake might be poor in the meanwhile.
The cough may be painful, use over the counter antitussives for symptomatic relief.

Not bacterial rhinosinusitis

Detailed Answer:
Hi...unlikely to be a acute bacterial rhinosinusitis in this age group. Even then as per current recommendations, we need to consider antibiotics only if fever is more than 10 days. With one day fever I wouldn't consider that. But if symptoms are relentless with facial pain or swelling, Iwould cconsider EBV infection too.
