Question: I am 30 years old, 5'9, 51kg, and have not had a natural period for 3 years. Tests during this time have not brought any conclusive reasons as to why this is, other than my low BMI/body fat. My efforts to put on weight is a struggle as I do not fluctuate at all. I have stopped all exercise (which I did not do huge amounts of before), but my weight seems to have dropped slightly despite increasing my calorie intake (muscle wastage)? I am naturally very lean. My reproductive organs have been checked - all is normal - the issue may lie with my pituitary glands. I have taken 2 types of HRT which induced bleeding, but it did not trigger any ovulation thereafter. I took my first round of Clomid 14/07-18/07 as we would like to start trying for a baby. Last weekend we had sex but yesterday 30/07, I went to the toilet to discover i was bleeding, and today it is very heavy, clotty, red. Much Like a period with period symptoms (lower back pain, cramps, headache). My understanding was I would not get a period for another couple of weeks (if we did not conceive), and that if I experienced any bleeding at all it would be lighter than what I am experiencing. I will monitor over the next 2 days but if it continues to be heavy (i am getting headaches and feel nauseous), I will contact my Dr. Any thoughts or advice would be appreciated. Thank you.

Thanks for writing to us with your health concern.
You need a complete evaluation regarding the reason for your absent natural cycles.
This should include -
An internal exam.
PElvic ultrasound scan done transvaginally
HOrmones - FSH, LH, TSH, Prolactin, OGTT, DHEAS, Free testosterone, Lipid Profile.
Please get this done, and only then proceed for fertility treatment.
Now, Clomid is never taken unmonitored.
Clomid should always be coupled with follicular monitoring. ( FM ).
FM includes alternate day or daily assessment of the egg follicle and tracking of ovulation with ultrasound scan done serially through the cycle.
This sounds like you had an early period, which sometimes happens when you take fertility hormones.
The best bet is to contact your doctor, have a scan done, and then plan your next round of Clomid with FM.
Take regular folic acid supplements.
Before taking Clomid, please get the assessment done as I suggested.
It is important to know where the problem lies, before attempting to treat it.
Being naturally lean is a plus, that should not affect your cycles, being overweight leads to irregular cycles, it is not as if you are anorexic.
Try and increase your protein intake, though.

Thank you for your quick reply.
I have had various internal transvaginal scans over the past 3 years, the most recent being on Monday.
I had the hormone tests in 2013 and then again in July last year (2 different gynos). There was nothing highlighted as being out of the ordinary there either. There was discussion about getting an MRI scan to check for tumour in the Pituitary Gland, but I don't have any symptoms of this possibility so have not pursued this.
I will speak with my Dr about the FM if she thinks I should continue on a second round and take on board your comments. My main concern was how heavy the bleeding is and how early this came about, but it sounds like this is not anything other than an early onset period. I am also concerned about my bones in the long term.

Could you upload the hormones tests that were done in the past ?
Also, an AMH level should be done to check if this is early onset menopause.
Only then should you be concerned about your bones ( osteoporosis ).
I do not think a pituitary MRI would yield anything if your prolactin levels were normal.
I would really wish to look at the hormonal tests done so far.
Yes, this seems like an early period.
Please NEVER take clomid blindly without FM alongwith it.
