What Causes Upper Right Abdominal Pain, Chronic Dry Cough And Elevated Liver Enzymes?

Question: Hi. I have been experiencing dull discomfort and tenderness in my right upper quadrant for months now. It comes and goes. Some days/weeks better than others. I also have a chronic dry cough. I've had 3 sets of blood. All fine apart from elevated Alanine Transferase. Beginning of Feb it was 69, beginning of March 73 and 11th April it was 113. CRP slightly elevated but GP said nothing of significance. I had an ultra sound in March which was normal. What could be causing this discomfort and elevated results aswell as cough. GP has given lansoprezole and gaviscon as he thinks could possibly be GERD - this has helped the cough slightly but not completely so no also having chest X Ray. I am slightly overweight 5 ft 9, 13 stone , 38 year old male but I have just started personal training. I don't drink excessively - nothing during the week - maybe a few glasses of wine or cocktails at the weekend. I had a colonoscopy in November - that was clear. I am concerned by this abdominal ache /liver results - what would you advise as next steps? Are the blood results much higher than you would expect. Many Thanks.

Hi and thank you fro asking!
I read carefully all your concerns and I can say as follows:
1.With regards to elevated AST and ALT level as I have wrote it earlier, it can be related with :
- hepatitis B, C this is why I suggest you to do serological exam
- overweight and fatty liver( perform abdominal ultrasound)

2. With regards to the pain that you are feeling on right upper abdomen it may be related with :
- gallbladder issues
- colon spasm due to IBS
Your pain is not related with liver test.
3.To my opinion you should perform the exams that I have mentioned above to achieve an accurate diagnosis.

Hi. Thanks for replying. As advised in my note I've already had an ultra sound and that was normal. My GP said if hepatitis then this would have shown in the scan and blood tests would be much worse. Please advise next steps. Thank you

Hi back,
Infection from hepatitis can not be detected from scan or ultrasound.
The most accurate test to diagnose hepatitis is serological test.
This is why I don't agree with your GP and recommend you to do the tests that I mentioned above.
