Question: Friend is 22 y/o male physically fit and healthy. Gets up this morning, goes to gym, then eats some chicken and rice, lies down in bed for a couple hours, and when he wakes has severe stomach pain, with sharp cramping that seems intermittent, every few minutes. It has him hunched over to ease the pain, either sitting or walking. Any ideas?

It is urgent and important to know if he has been diagnosed ever for peptic ulcer disease or gastritis. The pain associated with gastritis is usually a burning pain, aggravated by staying for a long time hungry, stress, some drugs and usually goes to the back. It is important to know if the pain has ever made him to wake up at early hours of the morning or not?

Another condition that presents as such is an inflammation of the pancreas, usually called acute pancreatitis. The pain in this case however is usually more severe.

I think he should be managed for peptic ulcer disease first. Eating regularly and avoiding any foods or drugs that aggravate pain is important. Not staying so long hungry. Medications that neutralize acids (antacids) and reduce acid secretion (proton pump inhibitors) can be used and are usually helpful.

In case these measures prove not helpful, he might have to see his primary care doctor for a proper clinical review. An abdominal ultrasound, CT scan and a fibroscopy to see if there are any lesions might be needed in the long run in case the initial advice fail to help.
