Posted on Fri, 28 Nov 2014
Question: I have spongey feet and recently this problem seems to have gotten worse. I was recently diagnosed with peripheral neuropathy. I have never been diabetic but I have been taking Lipitor for a long time and also Dilantin. I am 75 years old. Do you know what causes spongey feet? Any suggested treatment?

Brief Answer:
It could be dilantin or lipitor induced neuropathy

Detailed Answer:
Hi,
Thanks for asking.

I am Dr. Prakash HM and I will be answering your query.

Based on your query, my opinion is as follows:

Spongy feet - the feeling of walking on cotton balls or on spongy ground is due to sensory interference during nerve conduction. Most often it is due to peripheral neuropathy.

There are numerous causes of neuropathy. Due to absence of history of diabetes and present medication - we can think of dilantin or statin induced neuropathy or nutritional deficiency neuropathy (vit B12, B1, B6 or E)

If you were my patient, I would advise as follows:
1. Soft shoes with good arch support
2. Avoid standing or walking on foot for long time, as you wont be able to appreciate pain or other feeling.
3. Always check your feet.
4. Swimming, biking and water aerobics are appropriate to avoid injury to feet.
5. I am not sure dilantin or statin or nutritional deficiencies is causing neuropathy. All of them together or separately could be the cause.
6. Nutritional supplementation with B-complex along with Vit E. Multi-vitamin supplements would be helpful.
7. Avoid alcohol.
8. Try foot stretching to improve circulation. It might help in improving sensation.
9. Avoid injuries, as it will be painless just like in diabetes. Healing could be faster. However, it could get complicated.
10. Wear specially designed shoes/ foot wear, even inside home.
11. Avoid falls, walk on even ground only and in well lit areas.
12. Never walk bare footed, even if you dont have diabetes.

Vitamin supplementation is essential.
Be careful about stopping dilantin, not sure why it was prescribed. Lipitor can also cause neuropathy, so discuss with your doctor regarding which could be probable cause. Nutritional causes also needs to be looked into.

Hope it helps.
Any further queries, happy to help again.

Take care.
