What Causes Severe Itching And Small Skin Lesions On Forearm And Shoulder?

Question: my wife is having sever itching and small skin lesions on forearms and shoulder (attached picture) since 3-4 days .
Past h : previously, last week she was having mild feverish feeling for which I gave her tab PCT and an antibiotic (Augmentin ). During this time she applied Mehendi on her left forearm some 6-7 days ago . later after 3days she started to feel mild itching on the area of mehendi with small skin lesions limited only to a small area . now the lesions have increased with increased itching (pictures attached)
later since yesterday she noticed the skin itching and lesions have developed even on the other forearm and shoulder where she had not applied any mehandi.
She also says she has been facing CONSTIPATION since few days.

kindly give me a diagnosis and treatment .

I have gone through your query and I have also gone through the photographs

I will keep a possibility of a "Drug Reaction" (maculopapular type), possibly to the oral antibiotic. This is quite common with penicillin group of drugs.
Another possibility that I would like to keep is that it is part of a "Viral Exanthem" (she had a history of fever which probably was viral in origin).

I would like to know whether there are any mucosal lesions? Oral mucosa?

I don't think that this rash is related to mehandi application for the simple reason that it is generalized.

If I was the treating doctor I would suggest to continue the Oral antihistamine that she is already taking. She can take it during the day.
You may add another oral antihistamine at night i.e Tab Cetrizine 10 mg.

Topically she may use a soothing lotion e.g Calamine, 3-4 times daily.

No,Oral steroids.
Can use a topical steroid if it is very itchy and calamine not helping much. However, if it is subsiding on calamine, No need to use even topical steroids.
How is the rash now since you last consulted me?
