What Causes Pelvic Pain Radiating Down To Inner Thighs?

Question: Hi. I am having pain in my pelvic region, both in front and lower back, and it is radiating down both my inner thighs. It is a really deep ache and does not come and go with movement. The only thing I have found that helps is to lay on an ice pack or put the ice pack on my lower abdomen (about where I would have menstrual cramps, but this is definitely not menstrual pain). It comes and goes and sometimes it goes away after icing just once while other times it lasts several days at a time. The ice provides relief for a while (maybe an hour or three) and then comes back. If I don't have an ice pack, the pain becomes unbearable to the point of tears and brings on nausea. I have been diagnosed with IBS in the past- could this be related/ This has been going on for years but has been getting worse the past couple months. This past week has been awful.

Have you had a history of endometriosis ever ?
Apart form IBS, the only other condition that I can put as a differential diagnosis is endometriosis.
Even that is less likely because it mostly causes lower back pain, lower abdominal pain is less common with endometriosis.

I think this is related with your IBS. However it is always better to rule out other gynecological issues like fibroids, a cyst and pelvic infection. I will order for complete internal evaluation ( per vaginum and per speculum ) and a pelvic ultrasound scan done transvaginally.

Thank you for your response. I have never been told I had endometriosis. A few months back, the pain was so bad a relative took me to the ER. They did an ultrasound w/ transvaginal as well as a ct scan- would either of these have diagnosed endometriosis? They did say I had two small fibroids (1cm and 2cm) but thought they were not large enough to be causing this type of pain. Would you concur? They called me two days later and said they thought I had a UTI but a followup with a gen practitioner said that the urinalysis was negative. Is there something urinary/ kidney related I should be looking at?

No doctor has said they thought the pain could be due to IBS- it was just something I was wondering. I am not sure if it even makes sense that IBS might cause pain that radiates all the way into both my thighs and I have never heard of this as a symptom of IBS. I just really need ideas how to track down this pain so I can get it to stop.

Endometriosis is mostly diagnosed clinically - on internal evaluation by a gynaecologist - maybe the ER personnel were not qualifed.
Again, it might show up on an ultrasound or MRI - CT is not very reliable to diagnose endometriosis.
A few tricky cases need a diagnostic laparoscopy to be picked up.
2 small fibroids of the size mentioned cannot cause such excruciating pain.
Urinary / kidney pain would be colicky, spasmodic, would come and go in waves.
Also, if the urinalysis was negative and imaging was fine, again that aspect is ruled out.
IBS DOES lead to pain, again not typically the type you have, but that is the only condition that can explain this , if endometriosis is rightly ruled out.
When did you last have your period - is there any corelation that this pain has with your period, in terms of dates ?
