Posted on Mon, 23 Jan 2017
Question: Hello I want to ask something about my girl 3 years old. 7 days before she passed an influence with an average body temperature which arrived at XXXXXXX 38.2 with cough and all other symptoms. Fortunately she is better now but yesterday's to she began frequently urinating and today she screams while and says it hurts. I guess this might be a sign of an infection. She does not have temperature and I started using an integrator with propolis and vitamine C. Since today is 31 december and i van not go to make the analysis, should I be worried? Do I have to check imidiatly

Brief Answer:
No need to check immediately - not an emergency

Detailed Answer:
Hi.... by what you say I feel that she has come out from a viral illness recently.

Regarding the current issue of pain during passing urine and that too without any fever, I put forth these possibilities.

1. Concentrated urine
2. Urinary tract infection
3. Worm infestation

To confirm the second possibility we need to do urine routine examination. But this is not an emergency and you can do it on coming Monday.

Regarding the concentrated urine I would like to suggest home remedy so that you can relieve her distress immediately.

I suggest that you give hee one glass of sugar water with at least 4 teaspoon of sugar in it and then followed by 2 to 3 classes of normal water. This will make her urinate profoundly and relieve the symptoms if they are due to concentrated urine.

If she is normal and not complaining of any symptoms by Monday, then you need not check the urine routine examination also.

Coming to the third possibility of worm infestation, I suggest that you give her Albendazole tablet 400mg as a single dose today and another 400mg after 10 days. If this is a prescription drug in your country then you will need one for it.

How can I make the difference between? This medicine is it dangerous? What if she doesn't have worm infestation?

Answered by Dr. Sumanth Amperayani (1 hour later)
Brief Answer:
This is not an antibiotic or dangerous medicine

Detailed Answer:
Hi.... coming to your queries one by one -

1. You can make the difference only if you see worms being passed in the stool. The other way to find out will be to check a stool routine examination.

2. This is not an antibiotic or medicine with dangerous unwanted side effects.

3. Even if she does not have a worm infestation, there is no harm in giving Albendazole. It is safe drug.

Thank you again. I have bought this in the pharmacy and he told me that it is a big dose. Should I give her all the flacon? I am affraid of using medicine but as far as you are telling me to use this I will. Can I use it now. It is 10 pm now here and tomorrow will be all closed so I have to go the day after to do the analysis. It is not that she might have any complications? Thank you and Happy New Year!

Suggestions on urine routine report

Detailed Answer:
Hi.... the urine routine test is normal except for the nitrites being positive. The leucocytes count normal and this is not suggestive of urinary tract infection.

As I had already suggested this is not urinary tract infection as there is no fever at all.

My Suggestions -

1. Do not start antibiotics immediately. Using antibiotics is not wrong, but the indication for using antibiotics should be proper.

2. Please get urine culture done before starting antibiotics. In this way if the urine culture is not suggestive of urinary tract infection we can always stop the antibiotics.

3. I would like to know from you what antibiotic your doctor had suggested and in what dose? Please get back to me with the current weight of the kid also.

Thank you againg. The doctor if the turn after seing the analysis told that the number of leucocites are too high so she gave me NEOCEFI 4 ml 2 times during the day for ten days. The doctor that follows the girls from her birth after seing the analysis told me that I should use amoxicilin 250 mg 3 times during the day. I don't Know what to do.

Brief Answer:
There is only one page which is shown as an image screenshot

Detailed Answer:
Hi..... I agree with both the doctors regarding the antibiotic. Both the antibiotics are one and the same in action and efficacy for urinary tract infection. You can use any one of them, but not both. But please start the antibiotic after giving urine for culture test.

If urine culture report is not suggestive of a significant growth, then we can stop the antibiotic.

Coming to the turning of the page in your uploaded image, it is showing only one page as a screenshot image. I am not able to turn the page.

You can even mail a pdf format of the file to YYYY@YYYY

Thank you! I forget to tell you that she is 17 kg and 3 years and 6 months old. I have made frequent lavadges with camomile and she seems more calm today. She continues doing frequently urine and small amount and also tells me that it hurts. Tomorrow morning I am going to make the culture and today i am not starting the antibiotic. Regarding the analysis this is the only page that they have send me. What other kind of analysis should I do tomorrow except the uriculture? Regarding this urine analysis you think that this are normal values cause the doctor terryfied me by telling that this is a huge infection and you should start immediately the antibiotic cure. Thank you sincerely. One last question should I give again the ALBEDANZOLE today?

Brief Answer:
Urine culture is enough - if given Albendazole yesterday - repeat after 10d

Detailed Answer:
Hi.....thank you for following my suggestions.

Coming to your queries one by one-

1. Urine culture alone is enough and as soon as you give the urine sample for culture - you can start antibiotic.

2. If you have given Albendazole yesterday 400mg... you need to repeat it only after 10. days. No need to give second dose today itself.

3. As far as the report goes, there's no need to panic.

4. The urine culture sample has to be collected as a midstream clean catch sample.

It’s important to collect a urine sample midstream. This means that kid should start urinating, then place the collection container underneath the genital area and then catch urine.

I wish your kid a speedy recovery. If you need any future medical consultation and suggestions, I will be glad to help. You can approach me at the following link.

Thank you again and the last question: As far as I understand I have to start giving her antibiotic anyway? So this situation can not pass without giving antibiotic? Should I start after before or after the exam result?

Brief Answer:
Suggestions on antibiotic

Detailed Answer:
Hi...
1. I am telling you to start the antibiotic because, you told something which I could not see in the report. You told that there were plenty of leukocytes in the report. This is the reason why I told you to start the antibiotic but only after the culture.

2. But as there is no fever at all, there is a second option also. The urine culture report will usually be ready by 48 hours, so you can wait for 48 hours without starting an antibiotic. I am telling this because, usually urinary tract infection will have high grade fever with chills and rigors.

3. Usually after giving the urine sample for culture you need not wait till the culture report is ready. You can always start the antibiotic and if the culture is significant then you can continue the full course of antibiotics and if the culture is negative then you can always stop antibiotics within 48 hours.

Hope I have made myself clear and I am willing to take up any more questions in this regard if you have. Please feel free to come back to me with your queries.

Brief Answer:
Suggestions on antibiotic dosages

Detailed Answer:
Hi....I prefer Cephalexin....the dose for genitourinary tract infection is 25-50mg/kg/day divided in 2 or 3 doses.

The kid is 17kg..that works out to 850 mg per day.

I suggest giving Syrup Neocef (250mg/5ml strength), 5ml thrice a day for 10 days.

If we give antibiotics we should give at the right dose. The dose of 4ml twice a day will be a lower dose. I suggest giving it at 5ml thrice a day. The strength of the medicine is also very important. It is available at 125mg/5ml strength too. Please make sure you buy the 250mg/5ml strength one.
