Question: Hi. I have been having very bad pain followed with burning in my stomach, It started a few years back and just got worse, I had an Endoscopy done recently and was told by the doctor that done it , There will come a time when you will not be able to eat, I had the report back saying that My stomach mucosa was thinning , I have been put on Pantoprazole 40mg every morning but this as made no difference, I am worried that this may come cancerous , She told me it was not curable , I do not know which way to turn , My mornings are the worse and the burning through the day, I have also noticed it affecting my bowel and Bladder, I already suffer from Pancreatic deficiency , Asbestoses, Colostomy , Abscess welded between Bladder and very bad Anxiety owing to being abuse has a child by a friend of the family. I live in the UK

Dear Doctor XXXXXXX I am new on the internet, I have managed to put the documentation of my Endoscopy on to my computer but have no idea on how to send them to you. I can only try and give you all the information on the Email.
Gastro report, Reason. Dysphagia, Co-Morbities, None. current medication ,Omerprazole 40mg od. ASA status, 1.
1 Lower oesophagus (2)images
2 stomach. upper body (1)image
3 Middle body, Generalised oedematous mucosa.4 biopsy taken and placed in 1 container,
4-antrim (1)image region Duodenium-normal
5 second part 1 image.
Diagnoses , Oesophagus, normal. Stomach -Gastritis (Hyperemic), Duodenium ,Normal.
Report comments, Pangastritis-Oedematous and a little erythematous , increase
omerprazole bd. Tertiaty contractions in oesophagus in keeping with some mild dysomotility.
CELLULAR PATHOLOGY REPORT.
SPECIMEN, Gastric body.
CLINICAL DETAILS.
On PPI. mild inflammation, ?H Pylori.
MACROSCOPY.
two pieces mucosa upto 3 mm.
Microscopy.
Biopsies of gastric body mucosa showing some mild reactive changes only. No evidence of inflammation, atrophy , or dysplasia and no H-pylori type organisms are seen.

Hello, Sir.

I can certainly understand your concern.

I have gone through your query in detail.

You possibly seem to have simple pan gastritis

Since the biopsy is negative for H.Pylori, it is a welcome sign.

All you need is simple acid suppression therapy.

Instead of Omeprazole, I recommend Esomeprazole which is more potent for 21 days.

Restriction of fatty, fried and spicy food stuff will help to prevent these symptoms.

Consume dietary fiber like Oatmeal daily.

Elevate the head end of the bed by 30 degrees to prevent acid reflux.

Once the therapy is over, you will feel better.

Thank you Doctor XXXXXXX I have taken in everything you have told me, I do raise my pillows to 3 high, I do not eat really any type of fatty foods, If I eat fruit it seems to trigger it off, I have been on Pantoprazole 40mg once a day now for over 12 days but no improvement, I woke up this morning and I was suffering dreadful burning and that was in the middle of my stomach and all round my mouth, I tried taking some Gaviscon but it got worse, I do not seem to have some thing that will stop the burning plus the pain, I also would like to ask you why do you thing the Doctor who done the Endoscopy told me I would not be able to eat at some stage, I was to shocked to ask her to explain to me this. I wonder if you could tell me if this will get better has she also told me there was no cure for this illness, Kind regards XXXXXXX

Get an esophageal mannometry study.

Hello, Sir.

I can certainly understand your concern.

I apologize for the delayed response.

Earlier in your endscopy report ,there is a mention about incoordinate oesophageal tertiary contractions which could reflect an esophageal dysmotility disorder.

But I don't see this an irreversible condition as your doctor might have thought causing hindrance to swallowing permanently.

For further evaluation , I advise you to get an esophageal manometry and 24 hr pH study.

This will give a clue to underlying motility disorder like achalasia cardiac.

Defitinitive treatment can include addition of drugs called calcium channel blockers.

Get these tests and update me with test results.
