What Causes Pain In The Lower Right Back And Groin Area?

default
Posted on Mon, 24 Oct 2016
Question: Hi,
I have been in pain since Wednesday. The pain is a throbbing, tender and extremely sore. The pain is in my lower right back and front groin area. It escalated Thursday to the point where I was in tears. I've had this several times over the years but not to this extreme, it would flare up then subside and I could feel the discomfort but it seems as if the inflammation or flare up would calm down.

When it became unbearable I went to the ER and they gave me valium and morphine and they did a cat scan. The cat scan showed nothing and the pain still persisted and I was sent home still in tears from the extreme pain because they couldn't find anything wrong. I couldn't walk, sit or move. This extreme tenderness is also accompanied by what feels like a bladder infection but I never test positive for infection. I do get relief from the pressure and tenderness when I urinate and or have a bowel movement. I've seen my primary care, and a urologist. I've had a kidney and bladder ultra sound.
In fact, after my bladder ultrasound I had the pain again for 3 days as if whatever this is was pressed and it triggered the inflammation and soreness again to the point that I took extra strength Tylenol to deal with the pain. But the ultrasounds nor the cystoscopy showed anything. I do have a history of endo metriosis. I thought after I had a full hysterectomy this pain would go away because it would always flare up during my menstrual cycle but here I am 3 years later I still don't know what this pain is from and It is becoming debilitating.
Could this be endometriosis still? What are your thoughts? What should I be doing?

Detailed Answer:
Hello
Thanks for trusting us with your health concern.
Yes it sounds very much like endometriosis.
I would recommend an MRI scan since it is much more sensitive than a CT scan for picking up endometriosis.
If that is negative, I would then recommend a diagnostic laparoscopy to rule out endometriotic deposits or any other cause for the debilitating pain that you are having.
All the best.

Thanks, I asked for the MRI but both my primary care and GYN feel I should just have the laparoscopy. In the meantime, I still have the pain & continue to see blood in my urine. (I had a full hysterectomy 3 years ago so I imagine this is not normal.)

Detailed Answer:
Well, an MRI is less invasive .
So you can get that done first.
A laparoscopy is much more invasive, compared to a simple radiological test.
But then, your doctors have the prerogative to take the final call.
Take care.
