What Causes Lower Abdominal Pain?

Question: Hi,
I have for about a month now been having a painful ache in my lower abdomen in my left side that goes to my lower back. It's a very heavy full ache feeling that is worse with activity, sitting up or bending over. In fact if I bend over I almost throw up. I am Not pregnant, I've had bloodwork done, urine tested, ultrasound, cat scan all fine.... waiting now for the mri result which I'm terrified will show nothing. The last time I went through a round of tests showing nothing at all I ended up having to have my fallopian tubes out from infection..it just never showed up on anything. This has gone on so long and I'm so desperate to know what it could be. I have a history of ovarian cysts but none showed up on the ultrasound or cat scan. Hoping the mri shows something. What do you think it could be??

Please give an additional information for me to assist you better:
Any relation with the menstrual cycle?
When were the last investigations done? When was the surgery for Fallopian tubes done? Did this help or increased the problem?
Any associated symptoms related to urination like burning, pain, frequency?
Any symptoms related to intestines like change in bowel habits, bloating, gurgling or anything else?
Any relieving factors?
Appetite/ sleep/ loss of weight?

The most probable causes with the history you have posted can be as follows:
Pain arising from the spine may be due to cord compression or any disease causing pressure on the sensory nerve roots.
Colitis or IBS that is irritable bowel syndrome.
Endometriosis.
Left ureteric obstruction,m although this might have been picked up by ultrasound or CT.
Left inguinal hernia, will show a bulge, impulse on coughing and will always be reducible.

I would advise you the following in such a situation:
Clinical evaluation and examination by a General Surgeon as a second opinion.
MRI of the thoracic and lumbar region for any positive findings.
Colonoscopy if colitis and such related things are diagnosed.
Test of stool, blood and urine - to be repeated of already done and please post reports if possible.

Nausea can be a reflex phenomenon as a response to any pain or severe discomfort.
Feeling full and feeling that something is blocking on bending over can be related to the colon or so or can be the nerve stretching or pinching on bending giving the feeling.

Any relation with the menstrual cycle? I started bleeding during my mid cycle but I take birth control pills all 7 days with no break during the month so I don't usually bleed. After bleeding for a few days it stopped.

When were the last investigations done? I've been seeing doctors for the last three weeks, regular doctor, obgyn, specialist.
When was the surgery for Fallopian tubes done? about two years ago.
Did this help or increased the problem? Helped
Any associated symptoms related to urination like burning, pain, frequency? No but when I push while going it feels very very uncomfortable which it didn't before.
Any symptoms related to intestines like change in bowel habits, bloating, gurgling or anything else? I get diarrhea then constipated back and forth and some days I'm fine and have normal functions.
Any relieving factors? none at all, lying down feels better but only a bit.
Appetite/ sleep/ loss of weight? Hard to sleep because of discomfort, no weight change. Very thirsty but urinate fine.

Thanks for your feedback.
The points to be noted are:
Pushing on urination is uncomfortable.
Diarrhea then constipation back and forth, some days fine.
Hard to sleep due to discomfort.

All these points indicate further evaluation for any colonic problems by Colonoscopy and an opinion of a Gastroenterologist, tests of stool, blood and urine.
If all the investigations are normal, IBS that is irritable bowel syndrome can be considered.
