Question: Hi so i have experienced joint pain, cramps, numbmess and tingling for the past months. I had gotten bloood work done and the doctor just told me my allergies and i have symptoms of RA. However, my joints do not swell at all so I am confused with what i may have. The first blood tests i had, i had low blood sugar, positive ANA, and low thyroid. However, after that i was seeing a new guy and we did have sexual contact a few times, i feel like this is when the tingling and numbness and joint pain XXXXXXX I am so terrified of having HIV or something to that extent. What do i do??? Last sexual contact with this dude was 4 months ago almost. I am very worried

I appreciate your concern

HIV fourth-generation test that identifies the viral protein HIV-1 p24 antigen, should be done , which appears in blood before the HIV antibodies are formed.
If positive, then an immunoassay that differentiates HIV-1 from HIV-2 antibodies should be advised.

An STD panel including HIV can be advised in consult with your doctor.

A positive ANA warrants that anti ds antibody should be done which is much more specific for SLE (Systemic Lupus Erythematosis).

For R.A clinical assessment is primary. Anti CCP antibody and RA factor would support the diagnosis. ESR and CRP levels should also be done. If diagnosed then treatment should be initiated with DMARDs like methotrexate before any joint deformity sets in. Please consult a rheumatologist or internal medicine doctor in this regard.

I am going to be tested for all of these problems. With this said, do these symptoms contribute to any main disease?? I also have low thyroid if i forgot to mention that. I also have allergies where i cant have gluten dairy or eggs and found this out recently. I cut these out of my diet for good though

It doesn't point to a single condition. One autoimmune condition can be associated with other autoimmune conditions especially involving the thyroid. Hashimoto thyroiditis should be worked up. It is 10 times more common in women. Anti TPO antibodies should be looked for in this regard. Levothyroxine supplementation with Thyroid function tests monitoring 6 weekly can then be commenced.

The gluten free diet is opted for in Celiac disease. For a proper diagnosis a combination of immunology and jejunal biopsy is used. Reintroduction of gluten 6 weeks prior to testing is required as in celiac with gluten free diet reversibility of villous atrophy and immunology normalization occurs.
