What Causes Intermittent Tingling Sensation In Limbs?

Question: I have tingling in my feet and hands that comes and goes. My TSH level continues to jump around from 4.4 to 2.3 so not sure that has anything to do with the symptoms.

Fasting results from April 7th:
Protein 7.6 g/dl, vitamin B12 290, Creatine 104mg, Cholesterol 203mg, TSH 4.248,
WBC 5.9 k/ul, Sodium 141, RDW 13%, LDL 109mg, Hemoglobin 15.9 g, Hematocrit
45.8%, HDL 62mg, Calcium 9.8, Potassium 4.

Taking Zoloft 50mg once daily. I have a mother and father with thyroid disease. Then on XXXXXXX 2nd my doctor wanted to do another TSH level without fasting and it came back today at 2.7. Last year when I saw an endocrinologist it was at 4.1 and he said I was subclinical with a simple goiter. This has all been very frustrating and I am starting to think the worst. No one in my family has MS or any other neurological disorders. My hair is thinning, I have gained weight mostly in my stomach, my skin is very dry. I do NOT have headaches, double vision or balance issues. I do have a pins and needles sensation in my hands and feet that comes and goes. It is worst if I have been crossing my legs or sitting funny. It is starting to give me panic attacks because no one seems to have an answer. My father went through this for years before they finally figured out what his problem was and that was after they almost diagnosed him with ALS. The last thing is last year at the endocrinologist he said I was low positive for the antibody but did not explain any of the results.

Also my internist is thinking it could be a B12 deficiency. So I am about to start taking B12 shots.

Thanks for consulting me at virtual clinic. I have carefully worked through your case, and I can realize your health worries.

I will come to every point of your case one by one & today will answer all of your worries.

You have a strong family history of Thyroid disease. Your skin is also dry, that happens due to above normal heat production as in hyperthyroidism ( and remember it is also a clinical reason for B12 deficiency).

Your TSH levels are decreased from 4.23 to 2.7. It is still in normal range but decreasing, shows may be you are going towards hyperthyroidism ( but I can NOT say right now that you are hyperthyroid).

**You asked whether or not TSH level fluctuations related to on and off tingling??
Yes, if we keep this fluctuation in mind, as going towards hyperthyroidism, knowing that you have problem of dry skin too, we can say with confidence that TSH fluctuation lead to Vit. B12 deficiency and on and off tingling sensations.

**You are less likely to be a victim of Multiple Sclerosis. The symptoms you are facing suggest this diagnosis least likely. But definitive diagnosis can only be made with MRI of the brain and spinal cord and CSF (cerebrospinal fluid) examination. For prophylaxis and prevention, I suggest you to test your serial Vit. D levels, as normal Vit. D level is clearly associated with reduced risk for MS. Have a reasonable sunlight exposure to make your body self sufficient in making its own vit. D.

**And for, Amyotrophic lateral sclerosis (ALS), We can't make a quick definitive diagnosis early in its presentation. Usually, a neurologist need many months to exclude all other possible diagnoses in a patient presenting with upper and lower motor neuron lesion signs and symptoms. So we have to keep an eye, after even starting you on B.12. But right now, it doesn't fit into the diagnosis of ALS because ALS presents more with a Motor Neuronal involvement. But you have Sensory Symptoms. If it had been ALS, just to list, some of the symptoms had been Foot drop (patient has a slapping gait), Cramping stiffness and wasting of some hand muscles, Wrist drop that doesn't allow daily tasks to be done properly, Slurred speech, Choking during a meal.

I want to reinforce the decision of your Internist regarding B12 shots. Its because, when a female patient above 35 years, comes to my clinic, I test her B12 levels and if it reveals between 200-500 pg/mL, I definitely go for a course of subcutaneous or intramuscular Vit. B12 shots.

Your liver is doing good. Your cholesterol level is a bit high but other Lipid profile are all superb. Start brisk walk 30min daily.

Calcium is good, shows your parathyroids are doing good too and kidneys and bones are working fine.

Use Capsule Perfectil 1 per oral daily for 4 months for hair thinness. It will also give glow to your skin and shine to your nails.

Weight gain can be explained as a side effect of taking SSRI class of drug, Zoloft.

I recommend you to discuss with your intern and start with take 1000 mcg subcutaneous B12 shot daily for 1 week. Then 1 weekly for 4 weeks. Then 1 monthly upto the time B12 levels come to the normal range. Then you may also shift to oral ( although that has less absorption). Start injectible B12 after a test dose subcutaneously, for possible allergy.
