Brief Answer:
Risperidone or Mirtazapine augmentation of SSRIs.

Detailed Answer:
Hello
Welcome to HCM.
I have gone through your query thoroughly and understand your concerns.

You are suffering from OCD which has unfortunately not responded to several drugs.

Now you are on Latuda. In my opinion, Latuda is not the best choice for OCD.
There is no strong research evidence of beneficial effects of Latuda in OCD.

If I would get to treat a case of OCD like you, my approach would be to add an antipsychotic drug like risperidone to SSRI (from among fluoxetine, paroxetine or fluvoxamine).

If there is no response to this regime after one month, I would add mirtazapine to SSRI.

OCD is a difficult to treat condition but an appropriate drug combination can help in many cases.

Along with drugs, psychological treatments like exposure and response prevention are also helpful and must be started under supervision.

Your mental confusion at night may be due to latuda.

I would suggest you to discuss these treatment strategies with your doctor.

Hope this helps.
Feel free to ask further.

I wish for your speedy recovery.
Thanks.

i feeel the confusion is durign the day i thought that would be from klonopin i have tried multiple ssris but never together with an antipsychotic latuda isa n antipsychotic risperadal did not work ui was too sedated even on a low dose the thoghts are 24 hours with urges does that still qualify for ocd if psychosis what would the treatment be with medication
when will my follow up question be answered thank you

Brief Answer:
solian or abilify.

Detailed Answer:
Sorry for late response.

Both latuda and klonopin can cause confusion.

Risperdal did not work for you.

You have these thoughts 24 hours with urges.
It would qualify for OCD if you know and understand that these thoughts are irrational and persist against your will.

You were too sedated with risperdal. In that case, low dose of solian (amisulpride) or abilify (aripiprazole) should help you (in combination with SSRI). These are also antipsychotics but will not sedate you.
(prescription drugs).

Hope this helps.

Regards.

Brief Answer:
Same drugs will help.

Detailed Answer:
Hi again.

I think this is OCD.

Even if it is psychosis, the same drugs will help you (solian or ability). Dose adjustments will be needed depending on how you respond.

sorry I kept you waiting for the answer.

Can the thoughts and urges be all waking hours with ocd and have false memories where you are not sure if the event happened or not or if it is true?
HOW LONG WILL MY QUESTION BE TO ANSWER THANK YOU
doctor
Answered by Dr. Shubham Mehta (2 hours later)
Brief Answer:
OCD can keep the person engaged whole day.

Detailed Answer:
Hi again.

An individual with the kind of obsessions you have, usually will review his or her memory to gain certainty about events if those have already occurred.

The person may think about a time when some bad/harmful event might have occurred and have a “memory” about that event.

The more an individual attempts to gain certainty about the memory, the more memory distrust emerges. This further precipitates obsessions.

OCD can engage the person for whole day. That is quite common.

I hope this answers your query.
It would be great if you could further elaborate your question so that I can give you a more appropriate answer.

Regards.
