What Causes Delayed Periods After Taking I Pill?

Question: I am 20yr old.i had unprotected sex so i took i pill. After dat my periods were delayed for 15days and now when they came its 15th day they have not stopped .i have abdominal cramps too.the blood is normal menstrual blood and not heavy.
is it because of hormonal imbalance

This may be due to the I pill leading to transient hormonal imbalances. However if there is bleeding for 15 days then it is better to get an examination and rule out other causes like a pregnancy and abortion [ may occur if i pill fails], ovarian cysts or other hormonal issues like prolactin and thyroid abnormalities. Also sometimes cervical polyps and excessive growth of uterine lining [ less likely in your case] may also cause a heavy period.

So as of now, I suggest that you get a pregnancy test done. If it is negative then you may take tablets like tranexamic acid to control bleeding. If it still persists then you may need an examination.

Bt i got my menses and the blood was of menstruation only. Its like i am still on my day 3. Implantation bleeding cannot be because it started 18 days after sex and i took i pill immediately within 2 hrs

Hello and welcome back,

In your first query you have written that it is 15 days and the bleeding has not stopped. I understand that the bleeding is heavy.

When I say the bleeding may be due to pregnancy I did not mean an implantation bleed. Since the periods were delayed beyond 15 days, it may be an abortion. In any case, for any woman with a history of unprotected sex and prolonged bleeding after that, we rule out a pregnancy despite the use of emergency pill. This is because, failure of pill and abortions are also one of the common causes and they can be easily ruled out by doing a simple urine test.

Please understand that the management changes as per cause. We cannot localise every cause from history or examination alone. We need to supplement the findings with tests. So doing a urine pregnancy test is a baseline investigation or the starting point. If it is negative then we go for more expensive tests like an ultrasound and hormone testing. Please do understand that we cannot subject a patient to all these tests and then later find out that it was an abortion that could have been easily detected by a simple urine test.

So please do not think that I am thinking that this may be only pregnancy, but as I have already mentioned, pregnancy may be one of the reasons.

However I would also say that the most likely cause is the I pill as you have taken it within 2 hours.

So as of now, take tranexamic acid to control bleed. If it still perisists then you will need to see a doctor and get examined for other causes. I strongly recommed a pregnancy test to rule it out as it is a basic investigation if the bleeding does not stop.

I understand the issue but I cannot misguide you and feel it is my duty to provide complete information.

Please do not worry, such irregular bleeding is common with the pill and most of the time stops with tranexamic acid.
