Hello!

Thank you for asking on HCM!

I understand your concern, and would like to explain that, facing your husband' s complains, it is necessary to perform a comprehensive differential diagnosis wouk up.

Different tests should be reviewed: cardiac ultrasound and cardiac enzymes (CK-MB, troponin), coupled with BNP test, chest X ray study, pulmonary function test and an arterial blood gas analysis (probably already performed), could rule out cardiac and pulmonary origin of chest pain and dyspnea.

A complete blood count, thyroid hormone levels, and a backbone imagine study are necessary too.

You need to discuss with an internist for a careful physical examination, a better review of all the performed and recommended tests.

Hope to have been helpful to you!

If you have concrete test results or need additional
support, please do not hesitate to ask me!

Best regards! Dr. Iliri

Dr.Sharka, thank youbut we have seen several qualified cardiologists with negative results, tho husband has a family history full of heart problems, some dead from this
So we are concerned about this too. Would like to get some input from an Internist or maybe and Endocronoligst. We are desperate for answers,so please give us all you can, from whichever Dr might have had a patient same symptoms. Funny part of this is he will actually have a better day occasionally. All answers will be appreciated. Thames

He wakes up tired.and gets so weak just taking a shower. Has a good appetite does not smoke& sleeps well most nights. He worked in timber(in woods) most his life.Is it possible something like Lymes disease or something of this nature?? Correction, his family has history of heart problems.Does any doctor there have any suggestions,we desperatly need answers!!Please! He has no overweight or cholesterol, high blood problems, hope that helps. Thank you very much for any help. XXXX

Brief Answer:
A comprehensive differential diagnosis is needed.

Detailed Answer:
Hi again dear XXXX!

I understand you are in a desperate situation, after so many medical tests performed and visiting several doctors.

As I said you before a comprehensive diagnosis by a careful internist should be done.

Thyroid, as well as adrenal function tests should be checked.

Your husband clinical status is compatible with several pathological conditions, including systemic, neurological, metabolic, and infectious diseases, etc.

A neurological status evaluation, several lab tests (liver and renal function tests, CPK, PCR, many serological tests for possible infection involvement, etc).

A Lyme disease should also be considered as well, as his epidemiological and symptomatology background raise strong suspicions.

Your husband health issues are really a challenge, and should be clarified by an experienced internist. It is in fact a diagnosis by exclusion.

I wish this diagnostic and exhausting process could come to an end.

Best regards! Dr. Iliri

Brief Answer:
You are welcome!

Detailed Answer:

Dear XXXX!
XXXXXXX may be right on Metoprolol. Sometimes beta-blockers do exert such an unwanted effect, especially when associated with other drugs like Flecainide, etc.
A cardiac ultrasound should be reviewed for this purpose, even the cardiac rhythm status. After a comprehensive review by the internist, a possible cutting should be done to find the exact and most appropriate therapy scheme. I recommend you to follow my prior advises.

Feel free to ask me whenever you need.

Greetings! Dr. Iliri
