Question: Hi my 4 month daughter has developed stomach infection. it started with diaorrhea with mucus and greenish stool 3 days ago.My baby was putting a lot of pressure to pass stool 4 to 5 times in a day. She seemed to put pressure like an adult would do during constipation.Showed to Pediatrician who has advised walamycin 1 ml thrice a day, econorm half XXXXXXX thrice a day and Z&D 12 drops one in morning and one at night. I have following queries:

-1 ml walamycim means how many drops exactly and till when should i continue to give walamycin?

- should all 3 medications be given one after the other or in equal gap of time?

-when should i stop all three medication. My daughter is passing lesser greenish stools that is twice in a day ( morning and night) and small quantity of yellow curdled stool during rest of day time

- how will I know when to stop giving all of the above 3 mentioned medication?

Brief Answer:
Seems like viral diarrhea - stop antibiotics

Detailed Answer:
Hi...Thank you for consulting in Health Care magic.

It seems your kid is having viral diarrhoea. Once it starts it will take 5-7 days to completely get better. Unless the kid's having low urine output or very dull or excessively sleepy or blood in motion or green bilious vomiting...you need not worry.

There is no need to use antibiotics like Walamycin unless there is blood in the motion. Antibiotics might worsen if unnecessarily used causing antibiotic associated diarrhoea.

I suggest you use zinc supplements (Z&D drops 1ml once daily for 14 days) & ORS (Each small packet mixed in 200ml of potable water and keep giving sip by sip) as hydration is very important and crucial part of treatment. If there is vomiting you can use Syrup Ondansetron (as prescribed by your paediatrician).

Coming to your queries -
1. 1ml is 16 drops to be very precise.
2. I suggest you stop antibiotic Walamycin as there's no blood in stools.
3. You can give rest of medicines together or with time gap... It doesn't matter.
4. Unless the kid's having low urine output or very dull or excessively sleepy or blood in motion or green bilious vomiting...you need not worry. Your kid is already recovering. Give zinc for total 14 days and Econorm for total 5 days.

Regarding diet - You can use cerelac...any flavour will do. Avoid fruit juices as they might aggravate diarrhea. You can give zinc supplements & ORS apart from normal vegetarian porridges & soups.

Thank you for a quick response Doctor. As follow up question i wanted to know is it advisable to give water or Cerelac to 4 month old baby. Am asking so as ours is a traditional family that belives in introducing solid food and water only after 6 months of age.

Also which brand ORS should i give to my baby.

Additionally She has developed a bad rash on her bottom. i am applyig flutibact every 3 hours on affected area since last 2 days as prescribed by doctor. Rash was red color 2 days ago now rash has become pinkish should i use any other cream or flutibact is good?

Brief Answer:
Suggestions on feeds

Detailed Answer:
Hi...coming to your queries directly..

1. No water or cerelac till 6 months of age.
2. If the baby is having meagre diarrhea no need for ORS also. Only exclusive breast feeding will suffice. Any brand of ORS will do, I prefer Rebalanz.
3. Pinkish turning butt rash means its healing. You can either continue using Flutibact or use Siloderm ointment without steroid.

Hi Doctor,

It has been over 12 days now my 4 month old baby is still getting dark green stool. The consistency is not very thick neither very watery. we have been giving her Z&D drops since 12 days and econorm half XXXXXXX 3 times a day.

She seems to be a happy baby otherwise doesnt have any other symptoms. However we notice she is got excessive drooling and constantly putting her fingers in mouth. What do you think the problem is?

Brief Answer:
It could be gastrocolic reflex

Detailed Answer:
Hi...Thank you for consulting in Health Care magic.
What your kid is having is not a diarrhea and is only a gastro-colic reflux. It is quite common for babies of this age group t pass small amount of diarrhea or loose stools soon after feeds. This need not be treated as diarrhea and especially antibiotics are not indicated.
Unless the kid's having low urine output or very dull or excessively sleepy or blood in motion or green bilious vomiting...you need not worry.
